目的：探讨CT引导下经皮肺穿刺活检的应用价值及并发症。方法：回顾性分析CT引导肺部病变穿刺活检的213例资料，并统计穿刺成功病例的细胞学、病理学结果及并发症。结果：213例中确定病理诊断的有207例，其中腺癌78例、鳞癌53例（其中低分化鳞癌12例）、透明细胞癌3例、查到癌细胞但未能分型14例、小细胞癌22例、复合细胞癌2例、纤维组织3例、结节病3例、肺结核6例和炎性假瘤23例；穿刺失败和因取材过少或误取而未明确诊断的有6例。发生气胸7例，少量咯血2例，血胸1例。穿刺活检总确诊率为97％。结论：CT引导下经皮肺穿刺活检是一种定位准确、安全可靠、阳性诊断率高的检测方法，少数患者出现并发症。
목적：탐토CT인도하경피폐천자활검적응용개치급병발증。방법：회고성분석CT인도폐부병변천자활검적213례자료，병통계천자성공병례적세포학、병이학결과급병발증。결과：213례중학정병리진단적유207례，기중선암78례、린암53례（기중저분화린암12례）、투명세포암3례、사도암세포단미능분형14례、소세포암22례、복합세포암2례、섬유조직3례、결절병3례、폐결핵6례화염성가류23례；천자실패화인취재과소혹오취이미명학진단적유6례。발생기흉7례，소량각혈2례，혈흉1례。천자활검총학진솔위97％。결론：CT인도하경피폐천자활검시일충정위준학、안전가고、양성진단솔고적검측방법，소수환자출현병발증。
Objective:To investigate the value and complication of CT-guided percutaneous lung biopsy. Meth-ods:Retrospective analysis of CT guided biopsy of lung lesions for 213 cases were performed. Results:Of 213 cases i-dentified diagnosis in 207 cases,of which 78 cases were adenocarcinoma,53 squamous cell carcinoma( one of 12 cases of poorly differentiated squamous cell carcinoma),three cases clear cell carcinoma,found in cancer cells but failed to type in 14 cases,small cell cancer 22 cases,2 cases complex cell carcinoma,3 cases fibrous tissue,3 cases sarcoido-sis,tuberculosis,and 6 cases inflammatory pseudotumor of the 23 cases;puncture due to material failure and too little access or misuse in 6 cases without a clear diagnosis. 7 cases happened pneumothorax,2 cases with a small amount of hemoptysis,hemothorax 1. The total rate of needle biopsy confirmed 97%. Conclusion:CT-guided percutaneous lung biopsy is an accurate,safe,reliable,with high diagnosis rate detection method,a small number of patients happen com-plications.
