目的:研究甲状腺癌组织中Bcl-2、p53和ki-67的表达及其临床意义。方法:采用免疫组织化学SP法检测甲状腺癌组织、甲状腺腺瘤和正常甲状腺中Bcl-2、p53和ki-67的表达情况。结果:Bcl-2、p53和ki-67的阳性表达见于甲状腺癌组织和甲状腺腺瘤中,正常甲状腺组织中未见三者的阳性表达。甲状腺癌组织中Bcl-2的阳性表达率明显高于腺瘤组织( P<0.05),甲状腺癌组织中p53的阳性表达率明显高于腺瘤组织(P<0.05),ki-67在甲状腺癌中的阳性表达率为71.3%,高于甲状腺腺瘤10.0%(P<0.05)。甲状腺未分化癌和滤泡状癌组织中Bcl-2的阳性率明显增高。有淋巴结转移和临床Ⅲ、Ⅳ期病例中,Bcl-2的阳性率明显增高。结论:Bcl-2、p53和ki-67在甲状腺癌发生发展中起重要作用,Bcl-2、p53和ki-67的表达可作为甲状腺癌预后的指标。

목적:연구갑상선암조직중Bcl-2、p53화ki-67적표체급기림상의의。방법:채용면역조직화학SP법검측갑상선암조직、갑상선선류화정상갑상선중Bcl-2、p53화ki-67적표체정황。결과:Bcl-2、p53화ki-67적양성표체견우갑상선암조직화갑상선선류중,정상갑상선조직중미견삼자적양성표체。갑상선암조직중Bcl-2적양성표체솔명현고우선류조직( P<0.05),갑상선암조직중p53적양성표체솔명현고우선류조직(P<0.05),ki-67재갑상선암중적양성표체솔위71.3%,고우갑상선선류10.0%(P<0.05)。갑상선미분화암화려포상암조직중Bcl-2적양성솔명현증고。유림파결전이화림상Ⅲ、Ⅳ기병례중,Bcl-2적양성솔명현증고。결론:Bcl-2、p53화ki-67재갑상선암발생발전중기중요작용,Bcl-2、p53화ki-67적표체가작위갑상선암예후적지표。
Objective:To investigate expressions of Bcl-2,p53 and ki-67 in thyroid carcinoma and their clinical significance. Methods:SP immunohistochemical method was used to assess the expressions Bcl-2,p53 and ki-67 in thyroid carcinoma( TC group),thyroid adenoma( TA group)and normal thyroid tissues( control group). Results:Bcl-2 was expressed in 57. 5% of the thyroid carcinoma,20. 0% of the thyroid adenoma,and non in normal thyroid tissues. The positive expression rate of Bcl-2 in the thyroid carcinoma was significantly higher than that in the thyroid adenoma(P<0. 05). The positive expression rate of p53 in the thyroid carcinoma was significantly higher than that in the thyroid adenoma(P<0. 05). The positive expression rate of ki-67 in the thyroid carcinoma was significantly higher than that in the thyroid adenoma(P<0. 05). In thyroid carcinoma,undifferentiated carcinoma,follicular carci-noma,carcinoma with positive lymph nodes,and tumors with stage Ⅲ andⅣ disease had significantly higher expres-sion of Bcl-2. Conclusion:Bcl-2,p53 and ki-67 play an important role in the development of thyroid carcinoma and the overexpression of them might be a possible prognostic factor of thyroid cancer.
