目的：评价MR弥散加权成像（ DWI）参数与宫颈癌肿瘤细胞组织学特点的相关性，为宫颈癌的预后评价与治疗方案选择提供理论依据。方法：对55例宫颈癌患者行DWI检查（ b值＝0和1000s/mm2），然后评价ADC值与肿瘤细胞组织学特点的相关性，组织学特点包括病理类型、分化程度及肿瘤细胞间质比例。结果：宫颈鳞癌的ADC值明显低于宫颈腺癌（P＜0.05），低分化组的ADC值明显高于高分化组（P＜0.05），肿瘤细胞间质比例＜70％组的ADC值明显高于≥70％组（P＜0.05）。宫颈癌的ADC值与其病理类型有一定相关性（r＝0.320）；ADC 值与宫颈癌的分化程度和肿瘤细胞间质比例呈轻度负相关（r＝-0.298，-0.312）。结论：MR弥散加权成像（ DWI）参数与宫颈癌的组织细胞学特点密切相关，有望为宫颈癌的预后评价与治疗方案的选择提供理论依据。
목적：평개MR미산가권성상（ DWI）삼수여궁경암종류세포조직학특점적상관성，위궁경암적예후평개여치료방안선택제공이론의거。방법：대55례궁경암환자행DWI검사（ b치＝0화1000s/mm2），연후평개ADC치여종류세포조직학특점적상관성，조직학특점포괄병리류형、분화정도급종류세포간질비례。결과：궁경린암적ADC치명현저우궁경선암（P＜0.05），저분화조적ADC치명현고우고분화조（P＜0.05），종류세포간질비례＜70％조적ADC치명현고우≥70％조（P＜0.05）。궁경암적ADC치여기병리류형유일정상관성（r＝0.320）；ADC 치여궁경암적분화정도화종류세포간질비례정경도부상관（r＝-0.298，-0.312）。결론：MR미산가권성상（ DWI）삼수여궁경암적조직세포학특점밀절상관，유망위궁경암적예후평개여치료방안적선택제공이론의거。
Objective:To evaluate the correlation between diffusion imaging parameters and tumor cell histological characteristics of cervical carcinoma in order to provide the theoretical basis for evaluation of prognosis and selection of treatment protocol. Methods:Fifty-five patients with cervical carcinoma were enrolled into this study. MR diffu-sion imaging was performed in all these cases with b values of 0 and 1000s/mm2 ,and the apparent diffusion coeffi-cient( ADC)values were measured. The correlation between ADC value and tumor cell histological characteristics of cervical carcinoma( including pathologic type,differentiation degree and ratio of tumor cell to tumor interstitium)was analyzed. Results:The ADC value of cervical squamous carcinoma was lower than that of cervical adenocarcinoma( P<0. 05). The ADC value of poorly-differentiated cervical carcinoma was higher than that of well-differentiated cer-vical carcinoma(P<0. 05). The group of <70% in ratio of tumor cell to tumor interstitium had higher ADC value than ≥70% group(P<0. 05). The ADC value had a correlation with pathologic type of tumor(r=0. 320),and had a negative correlation with differentiation degree or ratio of tumor cell to tumor interstitium(r= -0. 298,-0. 312). Conclusion:The DWI parameter is closely correlated with the tumor cell histological characteristics of cervical carci-noma and promise to provide the theoretical basis for evaluation of prognosis and selection of treatment protocol.
