目的：观察新辅助化疗在结直肠癌中的临床疗效。方法：收集2008年6月-2012年6月住院患者资料。对62例进展期结直肠癌患者行术前全身化疗4个周期（ FOL FOX4方案），并于术后2周行辅助化疗2个周期（ FOL FOX4方案）。结果：经新辅助化疗，病人肿瘤平均直径缩小了32.5％，肿瘤的TNM分期得到降低。本组62例患者行4周期化疗后行腹腔镜辅助下结直肠癌根治术，手术顺利，无中转开腹，平均手术时间180min（120-240min），平均术中出血150ml（100-300ml），术后胃肠功能恢复时间1-2d，平均住院时间10d （8-14d），术中及术后无严重并发症。本组中有3例因术后并发肝肺转移于术后2年内死亡，1例脑转移死亡，失访5例，余53例均存活至今未发现局部复发、远处转移。结论：新辅助化疗在结直肠癌的治疗中，使肿瘤分期得以降期，不同程度缩小了肿瘤体积，减少局部复发，对中晚期结直肠癌有积极意义。
목적：관찰신보조화료재결직장암중적림상료효。방법：수집2008년6월-2012년6월주원환자자료。대62례진전기결직장암환자행술전전신화료4개주기（ FOL FOX4방안），병우술후2주행보조화료2개주기（ FOL FOX4방안）。결과：경신보조화료，병인종류평균직경축소료32.5％，종류적TNM분기득도강저。본조62례환자행4주기화료후행복강경보조하결직장암근치술，수술순리，무중전개복，평균수술시간180min（120-240min），평균술중출혈150ml（100-300ml），술후위장공능회복시간1-2d，평균주원시간10d （8-14d），술중급술후무엄중병발증。본조중유3례인술후병발간폐전이우술후2년내사망，1례뇌전이사망，실방5례，여53례균존활지금미발현국부복발、원처전이。결론：신보조화료재결직장암적치료중，사종류분기득이강기，불동정도축소료종류체적，감소국부복발，대중만기결직장암유적겁의의。
Objective:To study the effect of neoadjuvant chemotherapy in colorectal cancer. Methods:Data of 62 patients with advanced colorectal cancer with preoperative systemic chemotherapy treated 4 cycles( FOL FOX4 ),and on the postoperative 2 weeks of chemotherapy for 2 cycles( FOL FOX4 )was analyzed. Results:After neoadjuvant chemotherapy,the tumor diameter reduced by 32. 5%,TNM staging of tumor reduced. This group of 62 cases treated by 4 cycles of chemotherapy were treated by laparoscopic assisted radical resection of colorectal carcinoma. All opera-tions were successfully performed for the 62 patients with a mean operative time of 180min(120-240min). The mean intraoperative blood loss was 150ml(100-300ml),and the time for bowel function recovery was about 1-2 days,the postoperative hospital stay was 10 days(8-14 days). No other complications occurred. There were 3 cases with post-operative liver and lung metastasis died within 2 year after operation,1 patient died of brain metastasis,lost in 5 ca-ses,53 cases were survived so far no local recurrence,distant metastasis. Conclusion:Neoadjuvant chemotherapy in colorectal cancer treatment,can decrease tumor stage,tumor volume,local recurrence,has positive significance.
