目的:探讨B型脑钠肽在慢性阻塞性肺疾病中的水平变化关系并进行分析。方法选取本院2013年1~12月收治的58例慢性阻塞性肺疾病患者的临床资料,分为肺动脉高压组30例与肺动脉压正常组28例,另取同期来院体检正常者30例作为对照组,监测三组患者血浆B型脑钠肽(BNP)值与动脉血气分析值,通过心脏超声测定肺动脉压力,收缩末期右心室内径的大小、其与左室内径比。结果肺动脉高压组其血浆BNP水平、右室收缩末期内径、右室/左室内径比均明显高于肺动脉压正常组(P<0.05);肺动脉高压组其肺动脉压力与血浆BNP值呈正相关(r=0.42),肺动脉压正常组其血氧分压与血浆BNP呈负相关(r=-0.31)。结论慢性阻塞性肺疾病中血浆BNP是右室功能不全、肺动脉高压的敏感性指标。

목적:탐토B형뇌납태재만성조새성폐질병중적수평변화관계병진행분석。방법선취본원2013년1~12월수치적58례만성조새성폐질병환자적림상자료,분위폐동맥고압조30례여폐동맥압정상조28례,령취동기래원체검정상자30례작위대조조,감측삼조환자혈장B형뇌납태(BNP)치여동맥혈기분석치,통과심장초성측정폐동맥압력,수축말기우심실내경적대소、기여좌실내경비。결과폐동맥고압조기혈장BNP수평、우실수축말기내경、우실/좌실내경비균명현고우폐동맥압정상조(P<0.05);폐동맥고압조기폐동맥압력여혈장BNP치정정상관(r=0.42),폐동맥압정상조기혈양분압여혈장BNP정부상관(r=-0.31)。결론만성조새성폐질병중혈장BNP시우실공능불전、폐동맥고압적민감성지표。
Objective To explore the change of B-type brain natriuretic peptide levels in chronic obstructive pulmonary disease. Methods Choosing the clinical data of 58 patients with chronic obstructive pulmonary disease from January 2013 to December 2013 and dividing them into pulmonary hypertension group(38 cases) and pulmonary artery pressure normal group(20 cases), another in the same period to normal hospital check-up 30 cases as control group, the monitoring of the three groups of patients with plasma B-type brain natriuretic peptide(BNP) value and arterial blood gas analysis, pulmonary artery pressure by cardiac ultrasound measurement, end systolic right ventricular diameter size, with the left interior diameter ratio. Results The plasma BNP levels, right ventricular end systolic diameter, right ventricular/left interior diameter of pulmonary hypertension group were significantly higher than pulmonary artery pressure normal group(P<0.05);the pulmonary artery pressure and plasma BNP value in pulmonary hypertension group was a positive correlation(r=0.42), partial pressure of blood oxygen and plasma BNP in pulmonary artery pressure normal group was a negative correlation(r=0.31). Conclusion Plasma BNP in chronic obstructive pulmonary disease is sensitive indexes of right ventricular dysfunction and pulmonary arterial hypertension.
