目的：探讨部分难治性癫痫(RE)在添加拉莫三嗪(LTG)或奥卡西平(OXC)或托吡酯(TPM)联合丙戊酸钠(VPA)治疗的临床疗效。方法累积收集2009年1月~2014年1月符合标准的212例难治性部分性癫痫患者，对照治疗3个月前癫痫发作频度，并对治疗2年结束后的疗效、不良反应(ADR)及安全性和脑电图进行自身分析对比。结果患者应用LTG或OXC或TPM联合VPA治疗RE 2年结束后，患者发作频率均比用药前明显减少，患者治疗结束后的总有效率分别为89%、88.5%、78.1%， ADR的发生率分别为9.6%、19.2%、21.9%，脑电图总的好转率分别为89.9%、78.5%、76.9%。结论 LTG或OXC或TPM联合VPA治疗RE患者的疗效确切，耐受性好，安全性高， ADR轻。
목적：탐토부분난치성전간(RE)재첨가랍막삼진(LTG)혹오잡서평(OXC)혹탁필지(TPM)연합병무산납(VPA)치료적림상료효。방법루적수집2009년1월~2014년1월부합표준적212례난치성부분성전간환자，대조치료3개월전전간발작빈도，병대치료2년결속후적료효、불량반응(ADR)급안전성화뇌전도진행자신분석대비。결과환자응용LTG혹OXC혹TPM연합VPA치료RE 2년결속후，환자발작빈솔균비용약전명현감소，환자치료결속후적총유효솔분별위89%、88.5%、78.1%， ADR적발생솔분별위9.6%、19.2%、21.9%，뇌전도총적호전솔분별위89.9%、78.5%、76.9%。결론 LTG혹OXC혹TPM연합VPA치료RE환자적료효학절，내수성호，안전성고， ADR경。
Objective To explores clinical effect of valproate (VPA) therapy combined with lamotrigine (LTG) or oxcarbazepine (OXC) or topiramate (TPM) in the treatment of refractory epilepsy (RE). Methods Cumulative collection 212 standard patients with refractory partial epilepsy patients from 2009 to 2014, the control treatment three months ago seizure frequency, and 2 years after the end of therapy, the adverse reaction (ADR) and the safety and brain electrical diagram own analysis and comparison. Results Application of lamotrigine and oxcarbazepine or patients topiramate combined with sodium valproate in treatment of refractory epilepsy after 2 years, patients with seizure frequency than before treatment was significantly reduced in patients after treatment, the total effective rate was 89%and 88.5%and 78.1%;the incidence of ADR was 9.6%, 19.2%, 21.9%;EEG overall improvement rate was. 89.9%, 78.5%, 76.9%.Conclusion Effect of lamotrigine or oxcarbazepine or topiramate combined with sodium valproate in the treatment of intractable epilepsy patients exactly, good tolerance, high safety, ADR light.
