目的：分析临床电视胸腔镜手术(VATS)治疗自发性气胸(SP)的应用情况，探讨单操作孔电视胸腔镜手术的临床应用，为胸腔镜技术的开展提供更大的方向。方法回顾性分析60例自发性气胸手术治疗患者的临床资料，其中单操作孔电视胸腔镜手术30例(A组)，传统电视胸腔镜手术(两个或两个以上操作孔)组30例(B组)，对两组患者的临床指标进行比较。主要是手术时间、术中出血量、胸导管留置时间、术后住院时间以及患者的满意度调查。结果两组患者手术均成功，手术时间上没有明显差别，术中出血量、胸导管留置时间、术后住院时间及患者的满意度有着明显的差别。结论单孔胸腔镜手术，比常规胸腔镜少一个切口，创伤将更小，是未来微创外科的一个发展方向。
목적：분석림상전시흉강경수술(VATS)치료자발성기흉(SP)적응용정황，탐토단조작공전시흉강경수술적림상응용，위흉강경기술적개전제공경대적방향。방법회고성분석60례자발성기흉수술치료환자적림상자료，기중단조작공전시흉강경수술30례(A조)，전통전시흉강경수술(량개혹량개이상조작공)조30례(B조)，대량조환자적림상지표진행비교。주요시수술시간、술중출혈량、흉도관류치시간、술후주원시간이급환자적만의도조사。결과량조환자수술균성공，수술시간상몰유명현차별，술중출혈량、흉도관류치시간、술후주원시간급환자적만의도유착명현적차별。결론단공흉강경수술，비상규흉강경소일개절구，창상장경소，시미래미창외과적일개발전방향。
ObjectiveTo analyze clinical effect of video-assisted thoracoscope surgery (VATS) in the treatment of spontaneous pneumothorax(SP) and explore clinical application of single hole of video-assisted thoracoscope surgery to find out the better application in clinical.MethodsRetrospective analyzing clinical data of 60 patients with spontaneous pneumothorax surgery, including single hole video-assisted thoracoscope surgery operation 30 cases (group A), the traditional video-assisted thoracoscope surgery (two holes) group of 30 cases (group B), the clinical indexes of two groups of patients were compared. Mainly operating time, intraoperative blood loss, chest catheter indwelling time, postoperative hospital stay, and the patient's satisfaction survey.ResultsThe patients of two groups were performed successfully, there was no significant difference on the operation time, intraoperative blood loss, chest catheter indwelling time, postoperative hospital stay and patients’ satisfaction has obvious difference.ConclusionSingle-hole thoracoscope surgery, less than a conventional suture of incision, the wound will be smaller, it is a development direction of the minimally invasive surgery in the future.
