目的:探讨兔肺内肿瘤射频消融( RFA)后残存肿瘤细胞增殖和新生血管的变化。方法采用组织块悬液注射法建立兔VX2肿瘤肺内种植模型,64只新西兰白兔随机分为对照组(n=10)和RFA组(n=54)。以控制电极展开范围、输出功率和治疗时间的方法,造成RFA后肿瘤残存。采用免疫组化法计算不同时间点残存肿瘤细胞Ki-67标记指数(Ki-67LI)和微血管密度(MVD)。结果对照组Ki-67LI 为45.3%±2.1%。 RFA组Ki-67LI在第1、3、5天分别为56.4%±3.4%、60.1%±4.1%和59.8%±2.4%,显著高于对照组(均 P <0.05);在第7、9、14和21天分别为45.4%±2.0%、46.2%±3.4%、45.1%±4.4%和47.8%±3.9%,与对照组比较差异无统计学意义。对照组MVD为(28.9±2.9)条。 RFA组MVD在第3、5、7天分别为(36.8±2.6)、(55.6±4.8)和(51.5±2.8)条,显著高于对照组(均P<0.05);在第1、9、14、21天分别为(27.0±2.8)、(29.2±3.2)、(30.0±2.8)和(28.8±3.1)条,与对照组比较差异无统计学意义。结论 RFA后肺内残存肿瘤出现细胞增殖和新生血管一过性升高现象。

Objective To observe the changes of proliferation and angiogenesis of residual tumor in rabbit lung after radiofrequency ablation ( RFA).Methods The model of VX2 tumor in rabbit lung was established by injection of tissue block suspension.64 New Zealand White rabbits bearing VX 2 tumor were assigned randomly to the control group (n=10) and the RFA group (n=48).During the RFA procedure, residual tumors were achieved by controlling the range of electrode expanding , output power and treatment time.At several points of time, Ki-67 labeling index (Ki-67LI) and microvessel density (MVD) of the residual tumors were calculated by immunohistochemical detection.Results Ki-67LI of the control group was 45.3%±2.1%.Ki-67LI of the RFA group at the first, 3 and 5 day were 56.4%±3.4%, 60.1%± 4.1%and 59.8%±2.4%respectively , significantly higher than that of the control group;however , at the seventh, 9, 14 and 21 day, they were 45.4%±2.0%, 46.2%±3.4%, 45.1%±4.4% and 47.8%± 3.9%respectively , no significant difference compared with the control group.The control group MVD was 28.9 ±2.9.MVD of the RFA group at third, 5 and 7 day were 36.8 ±2.6, 55.6 ±4.8 and 51.5 ±2.8 respectively, significantly higher than that of the control group;however, at the first, 9, 14 and 21 day were 27 ±2.8, 29.2 ±3.2, 30 ±2.8 and 28.8 ±3.1 respectively, no significant difference compared with the control group.Conclusions The proliferation and angiogenesis of pulmonary residual tumor exhibit a transient increase phenomenon after RFA.
