目的探讨不同术式对于晚期癌症患者的治疗效果。方法以我院2009年1月～2012年7月间收治的83例晚期胃癌且可追溯患者为研究对象，采用回顾性研究的方法，按其手术方式，将姑息手术切除治疗者归为姑息切除组，将改道及探查手术治疗者归为非切除组，比较两组患者术后卫生经济学指标及生活质量的差异。结果研究结果显示，在卫生经济学指标的比较中，姑息切除组患者年化入院次数和年疾病花费低于对照组，在术后生活质量的比较中，姑息性切除组患者在心理健康、营养情况和活动能力方面得分高于非切除组，在癌性疼痛方面得分低于非切除组，其差异皆有统计学意义（P＜0.05）。多因素分析结果显示，姑息性切除手术可提高晚期癌症患者生活质量。结论姑息性手术可以有效降低患者术后就诊频率，减轻经济负担，并提高患者的生活质量，应在条件允许的情况下，尽量选择姑息性手术治疗。
목적탐토불동술식대우만기암증환자적치료효과。방법이아원2009년1월～2012년7월간수치적83례만기위암차가추소환자위연구대상，채용회고성연구적방법，안기수술방식，장고식수술절제치료자귀위고식절제조，장개도급탐사수술치료자귀위비절제조，비교량조환자술후위생경제학지표급생활질량적차이。결과연구결과현시，재위생경제학지표적비교중，고식절제조환자년화입원차수화년질병화비저우대조조，재술후생활질량적비교중，고식성절제조환자재심리건강、영양정황화활동능력방면득분고우비절제조，재암성동통방면득분저우비절제조，기차이개유통계학의의（P＜0.05）。다인소분석결과현시，고식성절제수술가제고만기암증환자생활질량。결론고식성수술가이유효강저환자술후취진빈솔，감경경제부담，병제고환자적생활질량，응재조건윤허적정황하，진량선택고식성수술치료。
Objective To analyze the effect of different surgery procedures in the treatment of late gastric cancer. Methods 83 patients with late gastric cancer were divided into palliative removed operation group and the no-removed operation group as their operation scheme.The difference of health-economic indicators and the quality of life between the two groups were compared with the retrospective study method. Results In the comparison of health-economic indicators,the frequency to be admitted hospital and the cost of the treatment every year of the palliative removed operation group were significant lower than the ones of no-removed operation group. Meanwhile,in the comparison of life quality,it was also more positive in the palliative removed operation group than the no-removed operation group.In the multiple factors analysis,palliative removed operation was a positive factor for the developing of life quality. Conclusion It would get a satisfactory effect to use the palliative removed operation in the treatment of late gastric cancer with the more positive health-economic effect ant the higher level of life quality.
