目的研究2型糖尿病患者血清SDF-1、VEGF水平与糖尿病视网膜病变的相关性,并分析影响血清SDF-1、VEGF水平的因素。方法选取2010-02-04内分泌科及眼科就诊的2型糖尿病患者59例,根据眼底情况分为无糖尿病视网膜病变(NDR)组20例、背景期糖尿病视网膜病变(BDR)组20例、增殖期糖尿病视网膜病变(PDR)组19例。另选取同时期体检中心健康体检者17例为对照(CON)组。采用双抗体夹心酶联免疫吸附法(ELISA法)定量测定血清SDF-1、VEGF水平。记录研究对象的病程、身高、体重、血压,测定空腹血糖(FPG)、糖化血红蛋白(HbA1c)、血清总胆固醇(TC)及肾功能指标。结果 BDR组、PDR组血清SDF-1水平比NDR组、CON组增高,均有统计学差异(均P<0.01);PDR组血清中SDF-1水平高于BDR组(P<0.01)。NDR组、BDR组、PDR组血清VEGF水平明显高于CON组,均有统计学差异(均P<0.01);NDR组、BDR组、PDR组3组两两比较血清VEGF水平无统计学差异(均P>0.05)。相关分析提示各组血清SDF-1水平与VEGF水平呈正相关(P<0.01),而血清SDF-1水平与患者病程及收缩压相关(均P<0.01),血清VEGF水平与患者病程、TC水平相关(均P<0.05)。结论2型糖尿病患者血清中SDF-1水平与血清VEGF水平呈正相关,两者与糖尿病视网膜病变的发生、发展关系密切,与糖尿病病程、收缩压、TC水平存在相关性。

목적연구2형당뇨병환자혈청SDF-1、VEGF수평여당뇨병시망막병변적상관성,병분석영향혈청SDF-1、VEGF수평적인소。방법선취2010-02-04내분비과급안과취진적2형당뇨병환자59례,근거안저정황분위무당뇨병시망막병변(NDR)조20례、배경기당뇨병시망막병변(BDR)조20례、증식기당뇨병시망막병변(PDR)조19례。령선취동시기체검중심건강체검자17례위대조(CON)조。채용쌍항체협심매련면역흡부법(ELISA법)정량측정혈청SDF-1、VEGF수평。기록연구대상적병정、신고、체중、혈압,측정공복혈당(FPG)、당화혈홍단백(HbA1c)、혈청총담고순(TC)급신공능지표。결과 BDR조、PDR조혈청SDF-1수평비NDR조、CON조증고,균유통계학차이(균P<0.01);PDR조혈청중SDF-1수평고우BDR조(P<0.01)。NDR조、BDR조、PDR조혈청VEGF수평명현고우CON조,균유통계학차이(균P<0.01);NDR조、BDR조、PDR조3조량량비교혈청VEGF수평무통계학차이(균P>0.05)。상관분석제시각조혈청SDF-1수평여VEGF수평정정상관(P<0.01),이혈청SDF-1수평여환자병정급수축압상관(균P<0.01),혈청VEGF수평여환자병정、TC수평상관(균P<0.05)。결론2형당뇨병환자혈청중SDF-1수평여혈청VEGF수평정정상관,량자여당뇨병시망막병변적발생、발전관계밀절,여당뇨병병정、수축압、TC수평존재상관성。
Objective To investigate the correlation of serum SDF- 1 and VEGF levels with diabetic retinopathy. Methods Fifty nine patients with type 2 diabetes were enrolled in the study, including 20 cases without diabetic retinopathy (NDR), 20 cases with background diabetic retinopathy (BDR) and 19 cases with proliferative diabetic retinopathy (PDR) , 17 healthy subjects were also included as control group. Serum SDF- 1 and VEGF levels were measured by enzyme- linked immunosorbent assay (ELISA). Blood pressure (BP), glycosylated hemoglobin A1c (Hb), and serum glucose, total cholesterol (TC), triglyceride (TG), and renal function were measured. Results Serum levels of SDF- 1 and VEGF were elevated in 3 subgroups of diabetic patients com-pared to controls (P<0.01). There were significant differences in serum SDF- 1 levels among NDR, BDR and PDR groups (P<0.01). The levels of SDF- 1 and VEGF were positively correlated with each other (r=0.626, P<0.01). Stepwise multiple regression analysis revealed that serum levels of SDF- 1 were correlated with course of diabetes and systolic BP (both P<0.01);VEGF levels were correlated with course of diabetes and TC (both P<0.05). Conclusion Serum SDF- 1 and VEGF are correlated with each other, both are closely related to the development and the disease courses of diabetic retinopathy.
