目的评价反序式回输方法在稀释性自体血回输中应用的效果.方法将90例择期骨科手术患者(预计出血量＞ 800ml,ASAⅠ～Ⅱ级)随机分为3组,每组30例.A组采用反序式回输法-自体血回输技术,B组采用顺序式回输法-自体血回输技术,C组不采取以上血液保护措施.三组在手术过程中有输血指征时,分别输入自体血或异体血.结果 A组只有1例输注异体血,B组只有3例输注异体血,而C组60％以上输注异体血.结论稀释性自体血回输能达到少输异体血,节约血源和减少输血风险的目的,而其中反序式回输方法效果更佳,值得临床推广.

Objective To evaluate the effect of reverse order method applied in dilutive autologous blood transfusion.Methods 90 patients undergoing elective surgery (estimated blood loss ＞ 800 ml,ASA Ⅰ-lⅡ level) were randomly divided into three groups (30 cases in each group):group A adopted reverse order method in autologous blood transfusion,group B adopted sequential order method,and group C did not take the bloodprotection methods above.During the operation,all the three groups were transfused autologous or allogeneic blood respectively when the blood transfusion indications appeared.Results Only 1 case was transfused allogeneic blood in group A,3 in group B,and more than 60％ patients in group C.Conclusions Dilutive autologous blood transfusion can reduce allogeneic blood transfusion,save blood,and decrease the risk of blood transfusion.The effect of reverse order method is better and worth being generalized clinically.
