目的:探讨原发性低颅压综合征(SIH)的临床特征及MRI表现,提高对本病的认识和诊断准确率。材料与方法回顾性分析22例确诊为SIH患者的临床及MRI资料,总结、分析SIH患者的临床表现及MRI特征。结果22例SIH患者临床出现体位性头痛22例(100%)、水平侧卧位CSF压<60 mm H2O (100%),颈部强直10例,听力障碍8例;MRI检查硬脑膜弥漫均匀强化22例(100%),脑下垂20例,脑室减小10例,静脉窦扩张7例。结论体位性头痛和CSF压减低是SIH的重要临床特征,硬脑膜弥漫均匀强化与脑下垂是SIH的重要MRI表现,SIH的临床及MRI均具有特征性表现,增强MRI是影像诊断SIH的重要手段。
目的:探討原髮性低顱壓綜閤徵(SIH)的臨床特徵及MRI錶現,提高對本病的認識和診斷準確率。材料與方法迴顧性分析22例確診為SIH患者的臨床及MRI資料,總結、分析SIH患者的臨床錶現及MRI特徵。結果22例SIH患者臨床齣現體位性頭痛22例(100%)、水平側臥位CSF壓<60 mm H2O (100%),頸部彊直10例,聽力障礙8例;MRI檢查硬腦膜瀰漫均勻彊化22例(100%),腦下垂20例,腦室減小10例,靜脈竇擴張7例。結論體位性頭痛和CSF壓減低是SIH的重要臨床特徵,硬腦膜瀰漫均勻彊化與腦下垂是SIH的重要MRI錶現,SIH的臨床及MRI均具有特徵性錶現,增彊MRI是影像診斷SIH的重要手段。

Objective:To investigate the clinical presentation and MRI characteristics of spontaneous intracranial hypotension (SIH) syndrome. Materials and Methods: Retrospective analysis of clinical presentation and MR imaging of 22 SIH patients, to summarize the characteristics of clinical presentation and MRI of (SIH). Results:Typical postural headache and the low CSF pressure (less than 60 mm H2O) ware found in the all patients. In the 22 patients, 10 cervical rigidity and 8 Hearing Disorders ware found. On MRI, all patients showed pachymeningeal diffuse enhancement, other findings included sagging brain (20), diminution of cerebral ventricles (10), enlargement of venous sinus (7). Conclusions:Postural headache and the low CSF pressure are the important clinical characteristics of SIH, diffuse enhancement of cerebral dura mater and sagging brain are the important MRI characteristics of SIH, enhanced MR was the important method in diagnosis of SIH.
