目的:探讨胫骨平台粉碎骨折合并胫骨干骨折的手术方式,分析外固定支架结合有限内固定治疗此类损伤的可行性及疗效。方法:采用外固定支架结合有限内固定治疗胫骨平台粉碎骨折合并胫骨干骨折22例,其中开放骨折15例,闭合骨折7例。开放性骨折先行清创、跟骨牵引,闭合性骨折行石膏固定或跟骨牵引,待全身情况稳定、患肢肿胀消退后行手术治疗。结果:22例均获随访,时间12~36个月。骨折平均临床愈合时间6个月。患肢力线正常,膝、踝关节功能满意(优良率72.7%)。结论:使用外固定支架结合有限内固定能整复关节面、维持骨折复位和下肢力线,对周围软组织影响小,促进关节早期功能锻炼,是治疗胫骨平台粉碎骨折合并胫骨干骨折的一种合理有效的方法。
目的:探討脛骨平檯粉碎骨摺閤併脛骨榦骨摺的手術方式,分析外固定支架結閤有限內固定治療此類損傷的可行性及療效。方法:採用外固定支架結閤有限內固定治療脛骨平檯粉碎骨摺閤併脛骨榦骨摺22例,其中開放骨摺15例,閉閤骨摺7例。開放性骨摺先行清創、跟骨牽引,閉閤性骨摺行石膏固定或跟骨牽引,待全身情況穩定、患肢腫脹消退後行手術治療。結果:22例均穫隨訪,時間12~36箇月。骨摺平均臨床愈閤時間6箇月。患肢力線正常,膝、踝關節功能滿意(優良率72.7%)。結論:使用外固定支架結閤有限內固定能整複關節麵、維持骨摺複位和下肢力線,對週圍軟組織影響小,促進關節早期功能鍛煉,是治療脛骨平檯粉碎骨摺閤併脛骨榦骨摺的一種閤理有效的方法。
목적:탐토경골평태분쇄골절합병경골간골절적수술방식,분석외고정지가결합유한내고정치료차류손상적가행성급료효。방법:채용외고정지가결합유한내고정치료경골평태분쇄골절합병경골간골절22례,기중개방골절15례,폐합골절7례。개방성골절선행청창、근골견인,폐합성골절행석고고정혹근골견인,대전신정황은정、환지종창소퇴후행수술치료。결과:22례균획수방,시간12~36개월。골절평균림상유합시간6개월。환지력선정상,슬、과관절공능만의(우량솔72.7%)。결론:사용외고정지가결합유한내고정능정복관절면、유지골절복위화하지력선,대주위연조직영향소,촉진관절조기공능단련,시치료경골평태분쇄골절합병경골간골절적일충합리유효적방법。
Objective:To study the effects of external fixator combined with limited internal fixation in treatment of Tibial plateau comminuted fracture combined with tibial shaft fractures.Methods:From Mar 2005 to Mar 2011,22 patients with Tibial plateau comminuted fracture combined with tibial shaft fractures were treated with external fixator combined with limited internal fixation. Results:Al the patients were fol owed up at an average of 24 months,and the average healing time was 6 months.16 patients achieved excel ent and good results(72.%).Conclusion:External fixator combined with limited internal fixation can fix the fragments of fracture effectively,and also permit early motion of the joint ,which can prevent stiffness of the knee and ankle.Therefore,It is one of an effective treatment method for Tibial plateau comminuted fracture combined with tibial shaft fractures.
