目的：通过我院骨科2000-2010年对278例胫腓骨开放性骨折伴软组织缺损的不同治疗方法的疗效，探讨一种更好的治疗方式；方法对278例胫腓骨开放性骨折伴软组织缺损的患者123例给予早期清创+石膏或牵引固定，二期植皮及皮瓣转位疗效和155例早期清创+外固定支架+负压封闭引流，二期植皮及皮瓣转位疗效的对比。结果第二种方法在住院时间，医疗费用，特别是感染率，截肢率明显降低；结论对于胫腓骨开放性骨折伴软组织缺损的患者早期清创+外固定支架+负压封闭引流，二期植皮及皮瓣转位是一种较好的治疗方法。

Objective: a good therapeutic method is found by analysis on the surgical treatment and curative effect of open fracture of the tibia and fibula with soft tissue defect of the 278 cases from 2000 to 2010 in our hospital. Methods:the 123 of 278 cases is treated by primary debridement + plaster immobilization or fixed traction and secondary stage dermatoplasty and transfer of skin flap, while the other cases by primary debridement + orthofix fixation + vacuum sealing drainage, secondary stage dermatoplasty and transfer of skin flap. Result: the cases treated by the second method not only take shorter hospital stays and hospitalization costs, but also have lower infection rate and amputation rate than the cases treated by the first method. Conclusions: for the cases with open fracture of the tibia and fibula with soft tissue defect, the second therapeutic method is better than the first method.
