目的:探讨硫酸镁对慢性阻塞性肺疾病急性加重( AECOPD)患者血浆脑钠肽( BNP)含量的影响。方法选择50例AECOPD患者,剔除肺心病患者,随机分为两组,每组25例。第一组常规治疗,第二组加用硫酸镁微泵注入。稳定期COPD患者20例作为对照组。分别在治疗前、治疗后7天用ELISA方法检测血浆 BNP水平、超声心动图测量肺动脉压,进行比较。结果①两组AECOPD患者治疗前血浆BNP水平、肺动脉压较对照组显著升高( P<0.01),两组AECOPD患者间无明显差异( P>0.05)。②经治疗后,两组AE-COPD患者BNP水平及肺动脉压均较治疗前下降(P<0.01);且硫酸镁组患者较常规治疗组下降更显著(P<0.01)。③两组AECOPD患者治疗后BNP含量及肺动脉压呈正相关关系。结论加用硫酸镁治疗AECOPD,能更有效的降低血浆BNP水平及肺动脉压,疗效显著。

목적:탐토류산미대만성조새성폐질병급성가중( AECOPD)환자혈장뇌납태( BNP)함량적영향。방법선택50례AECOPD환자,척제폐심병환자,수궤분위량조,매조25례。제일조상규치료,제이조가용류산미미빙주입。은정기COPD환자20례작위대조조。분별재치료전、치료후7천용ELISA방법검측혈장 BNP수평、초성심동도측량폐동맥압,진행비교。결과①량조AECOPD환자치료전혈장BNP수평、폐동맥압교대조조현저승고( P<0.01),량조AECOPD환자간무명현차이( P>0.05)。②경치료후,량조AE-COPD환자BNP수평급폐동맥압균교치료전하강(P<0.01);차류산미조환자교상규치료조하강경현저(P<0.01)。③량조AECOPD환자치료후BNP함량급폐동맥압정정상관관계。결론가용류산미치료AECOPD,능경유효적강저혈장BNP수평급폐동맥압,료효현저。
Objective To evaluate the effect of magnesium sulfate on plasma BNP in patients with AECO-PD. Methods 50 patients with AECOPD without any clinical evidence of cor pulmonale were randomly divided into two groups. The first group was treated with conventional therapy, and the second group was additionally treated with magnesium sulfate. Another 20 patients with stable COPD were selected as the control group. Plasma BNP concentra-tions were measured on admission and on the seventh days by using enzyme immunoassay. Echocardiography was per-formed by an ultrasound specialist to assess RV function and pulmonary artery systolic pressure ( PASP) on the first and seventh days. Results ① Compared with the control group, the plasma BNP concentrations and PASP signifi-cantly elevated in the AECOPD patients (P<0. 01). ② Plasma BNP concentrations and PASP in patients with AE-COPD decreased significantly after the treatment, and the decrease was more pronounced in the second group than in the first group (P<0. 01). ③The BNP levels were significantly positively correlated with PASP in patients with AE-COPD. Conclusion Magnesium sulfate can decrease the levels of plasma BNP and PASP obviously in the treatment of patients with AECOPD, with good curative effect.
