目的 探讨封闭式吸痰在机械通气中的临床价值.方法 2008年1月至2011年12月我院呼吸科收治的需要机械通气的患者65例随机分为对照组35例和观察组30例,对照组采用传统的开放式吸痰管理,观察组使用封闭式吸痰管理系统.总结对比两组患者的血氧饱和度SPO2及心率变化,对比两组患者镇静评分.结果(1)吸痰前及吸痰后两组患者的SPO2差异无显著性(P>0.05),吸痰时观察组的SPO2[(90±0.21)%]明显较对照组[(71±2.50)%]高,差异有显著性(P<0.05).(2)吸痰前及吸痰后两组患者的心率差异无显著性(P>0.05),吸痰时观察组的心率[(93.8±1.1)bpm]明显较对照组[(118±0.75)bpm]稳定,差异有显著性(P<0.05).(3)观察组患者镇静评分(0.9±0.7)较对照组(1.87±0.8)低,差异有显著性(P<0.05).结论 在机械通气治疗中采用封闭式吸痰是安全有效的,值得推广.

목적 탐토봉폐식흡담재궤계통기중적림상개치.방법 2008년1월지2011년12월아원호흡과수치적수요궤계통기적환자65례수궤분위대조조35례화관찰조30례,대조조채용전통적개방식흡담관리,관찰조사용봉폐식흡담관리계통.총결대비량조환자적혈양포화도SPO2급심솔변화,대비량조환자진정평분.결과(1)흡담전급흡담후량조환자적SPO2차이무현저성(P>0.05),흡담시관찰조적SPO2[(90±0.21)%]명현교대조조[(71±2.50)%]고,차이유현저성(P<0.05).(2)흡담전급흡담후량조환자적심솔차이무현저성(P>0.05),흡담시관찰조적심솔[(93.8±1.1)bpm]명현교대조조[(118±0.75)bpm]은정,차이유현저성(P<0.05).(3)관찰조환자진정평분(0.9±0.7)교대조조(1.87±0.8)저,차이유현저성(P<0.05).결론 재궤계통기치료중채용봉폐식흡담시안전유효적,치득추엄.
Objective To explore the clinical value of closed suctioning during mechanical ventilation.Methods We retrospectively reviewed the cases admitted in respiratory department of our hospital from January 2008 to December 2011 who required mechanical ventilation,and they were randomly divided into two groups: control group with 35 cases using the traditional open suctioning management;observation group with 30 cases using closed suction management system.The oxygen saturation SPO2 and heart rate changes in two groups were summed up and compared.Sedation scores of both groups were also compared and analyzed.Results ① In observation group and control group,the blood oxygen saturation SPO2 levels before and after suctioning had no significant difference(P> 0.05),while the oxygen saturation of observation group SPO2[(90 ± 0.21)%]during suctioning was significantly higher than that of the control group SPO2[(71 ± 2.50)%]with statistical significant difference(P < 0.05).②The heart rate showed no significant difference(P> 0.05)before and after suctioning,but during suctioning progress,the heart rate observation group[(93.8 ± 1.1)bpm]was significantly more stable than that of the control group[(118 ±0.75)bpm],the difference was statistically significant(P< 0.05).③Sedation score of observation group(0.9 ± 0.7)was much lower than that of control group(1.87 ± 0.8),with statistical significant difference(P<0.05).Conclusions The use of closed suction during mechanical ventilation is safe,effective,and worthy of promotion.
