目的：比较动力髋螺钉（ DHS）和股骨近端髓内钉（ PFNA）两种内固定治疗不稳定型股骨粗隆间骨折的临床疗效。方法自2008年5月至2013年4月间对95例老年股骨粗隆间不稳定型骨折患者进行回顾性研究，患者分为DHS组45例与PFNA组50例，比较两组的手术时间，术中出血量，下地行走时间，骨折愈合时间及髋关节功能等数据。结果 PFNA组和DHS组在手术时间，术中出血量，下地行走时间有显著差异，而在骨折愈合时间无明显差异，对于不稳定型股骨粗隆间骨折， PFNA组的髋关节功能优良率高于DHS组。结论对于不稳定的股骨粗隆间骨折，PFNA系统具有出血少，手术时间短，恢复快等的优点。
목적：비교동력관라정（ DHS）화고골근단수내정（ PFNA）량충내고정치료불은정형고골조륭간골절적림상료효。방법자2008년5월지2013년4월간대95례노년고골조륭간불은정형골절환자진행회고성연구，환자분위DHS조45례여PFNA조50례，비교량조적수술시간，술중출혈량，하지행주시간，골절유합시간급관관절공능등수거。결과 PFNA조화DHS조재수술시간，술중출혈량，하지행주시간유현저차이，이재골절유합시간무명현차이，대우불은정형고골조륭간골절， PFNA조적관관절공능우량솔고우DHS조。결론대우불은정적고골조륭간골절，PFNA계통구유출혈소，수술시간단，회복쾌등적우점。
Objective To compare the clinical curative effects of the dynamic hip screw ( DHS ) and the proximal femoral nail ( PFNA ) in the unstable intertrochanteric fracture of the femur .Methods Between May 2008 to April 2013,95 elderly patients of the unstable femoral intertrochanteric fracture in our hospital were enrolled in the study .All the cases were randomly divided into two groups:45 cases were in the DHS group and 50 cases were in the PFNA group .The operation time , intraoperative blood loss , walking time, fracture healing time, the hip joint function and other data of the two groups were compared . Results There were significant differences in the operation time , the intraoperative blood loss , and the walking time between the PFNA group and the DHS group; but there was no obvious difference in the fracture healing time .As for the hip function restoration of the unstable intertrochanteric fracture , the PFNA group was better than the DHS group .Conclusion In the unstable intertrochanteric fracture , the PFNA system has the advantages of less bleeding , short operation time , and fast recovery .It is worthy of promotion.
