目的:分析人工关节置换围术期患者实施规范化疼痛教育的效果,探讨其作用和必要性。方法不同于传统的疼痛教育模式,采用规范化的疼痛教育和评估,包括以下内容:(1)入院后8 h内完成首次疼痛评估与疼痛教育;(2)术前每天对患者进行评估和强化;(3)术后疼痛评估与疼痛教育和突发轻度及轻度以上的疼痛等对行人工关节置换的患者在围手术期进行有计划的面对面教育,经过护患共同学习;(4)住院期间不断强化疼痛方面的知识与互动;(5)出院时给予疼痛方面的指导;(6)在坚持无痛的原则下,形成不间断的疼痛跟踪。结果患者在疼痛理念、疼痛认知等方面获得较大提高。术后疼痛评分、睡眠时间、功能康复锻炼、疼痛满意度等方面有显著改善。结论规范化疼痛教育有利于人工关节置换患者的疼痛理念改变,帮助患者主动减轻术后疼痛,对患者术后康复,提高患者满意度有明显促进作用。

목적:분석인공관절치환위술기환자실시규범화동통교육적효과,탐토기작용화필요성。방법불동우전통적동통교육모식,채용규범화적동통교육화평고,포괄이하내용:(1)입원후8 h내완성수차동통평고여동통교육;(2)술전매천대환자진행평고화강화;(3)술후동통평고여동통교육화돌발경도급경도이상적동통등대행인공관절치환적환자재위수술기진행유계화적면대면교육,경과호환공동학습;(4)주원기간불단강화동통방면적지식여호동;(5)출원시급여동통방면적지도;(6)재견지무통적원칙하,형성불간단적동통근종。결과환자재동통이념、동통인지등방면획득교대제고。술후동통평분、수면시간、공능강복단련、동통만의도등방면유현저개선。결론규범화동통교육유리우인공관절치환환자적동통이념개변,방조환자주동감경술후동통,대환자술후강복,제고환자만의도유명현촉진작용。
Objective To analyze the effects of the regularized pain education on the patients undergoing the total hip or knee arthroplasty and to discuss its necessities .Methods Regularized pain evaluation, different from that of the traditional pain education mode was carried out as follows : (1) The first evaluation and pain education was fulfilled in 8 hours after the hospitalization;(2) the daily evaluation was repeated and the education was strengthened;(3) the post-operative pain was evaluated and the face-to-face pain education was carried out right after the total hip /knee arthroplasty surgeries to teach the patients dealing with the mild or more severe pain .( 4 ) The patients'comprehension on post -operative pain was exchanged and strengthened every day during hospitalization . ( 5 ) The guidence on pain administration was given at the time of discharging .( 6 ) Pain status was followed up regularly after the discharging .Results The patients'knowledge , as well as the comprehension on postoperative pain was significantly strengthened and their post -operative pain score , sleeping duration , rehabilitation practicing level and pain comfortablity were all increased simultaneously .Conclusion Regularized pain education contributs to change patients comprehension on post -operative pain , also helps reducing the post -operative pain , promoting the post -operative rehabilitation and improving the patients'satisfaction in the perioperative period .
