目的：研究扩散加权成像（ DWI）在星形细胞瘤和脑炎中的显示特征以及疾病鉴别诊断中的价值。方法：回顾2008年6月至2013年6月我院收治的采用MRI、DWI进行检查的33例星形细胞瘤和37例病毒性脑炎患者的临床病例。对2组患者的DWI显示进行分析，并对DWI的表观和相对表观的扩散系数ADC值、rADC值进行分析。结果：2组患者MRI表现有显著性区别；星形细胞瘤的ADC、rADC值分别为（1.55±0.37）×10-3 mm2/s和（1.86±0.62）×10-3 mm2/s，显著性高于病毒性脑炎的（1.07±0.29）×10-3 mm2/s和（1.31±0.35）×10-3 mm2/s，P＜0.01。结论：毒性脑炎和星形细胞瘤患者DWI的表观和相对表观的扩散系数ADC、rADC值有显著性差异，可作为鉴别诊断的依据。
목적：연구확산가권성상（ DWI）재성형세포류화뇌염중적현시특정이급질병감별진단중적개치。방법：회고2008년6월지2013년6월아원수치적채용MRI、DWI진행검사적33례성형세포류화37례병독성뇌염환자적림상병례。대2조환자적DWI현시진행분석，병대DWI적표관화상대표관적확산계수ADC치、rADC치진행분석。결과：2조환자MRI표현유현저성구별；성형세포류적ADC、rADC치분별위（1.55±0.37）×10-3 mm2/s화（1.86±0.62）×10-3 mm2/s，현저성고우병독성뇌염적（1.07±0.29）×10-3 mm2/s화（1.31±0.35）×10-3 mm2/s，P＜0.01。결론：독성뇌염화성형세포류환자DWI적표관화상대표관적확산계수ADC、rADC치유현저성차이，가작위감별진단적의거。
Objective:To explore the differential diagnosis value of diffusion weighted imaging ( DWI) on the viral encephalitis and low grade astrocytomas .Methods:37 cases with acute or subacute viral encephalitis and 33 cases with low grade astrocytomas were all diagnosed by MR imaging and DWI .The apparent diffusion coefficient ( ADC ) values and relative apparent diffusion coefficient ( rADC ) values in the two groups were calculated and analyzed .Results:The mean ADC values and rADC values were (1.55 ±0.37) ×10 -3 mm2/s and (1.86 ±0.62) ×10 -3 mm2/s respectively in the viral encephalitis; were (1.07 ±0.29) ×10 -3 mm2/s and (1.31 ±0.35) ×10 -3 mm2/s respectively in the low grade astrocytomas .There was significant difference in the mean ADC values ( P<0.01 ) and rADC values ( P<0.01 ) in the two groups .Conclusion:The ADC value and rADC value of DWI play a important roles in the differential diagnosis between viral encephalitis and low grade astrocytomas .
