目的：观察采用玻璃体切除术联合白内障切除术治疗增殖性糖尿病视网膜病变（ proliferative diabetic retinopathy ，PDR）的疗效。方法：我院2008年6月至2010年6月采用玻璃体切除术联合白内障切除术治疗PDR患者68例共74眼，对患者手术前后视力状况和并发症等进行统计分析。结果：Ⅵ期患者的硅油填充率显著大于Ⅳ期和Ⅴ期患者（ P＜0.01）；随访52例患者（71.27％）的视力得到了提升，Ⅳ期、Ⅴ期、Ⅵ期患者术后的视力增加率为100.00％、90.48％和61.22％；术后患者一次性视网膜复位成功率为100％。术后玻璃体出血1眼，术后后发性白内障8眼，填充硅油的患者有7眼出现高眼压，3眼出现硅油乳化。结论：玻璃体切除术联合白内障切除术可有效地对PDR进行治疗，改善患者的视力和减少再次白内障切除术的几率。
목적：관찰채용파리체절제술연합백내장절제술치료증식성당뇨병시망막병변（ proliferative diabetic retinopathy ，PDR）적료효。방법：아원2008년6월지2010년6월채용파리체절제술연합백내장절제술치료PDR환자68례공74안，대환자수술전후시력상황화병발증등진행통계분석。결과：Ⅵ기환자적규유전충솔현저대우Ⅳ기화Ⅴ기환자（ P＜0.01）；수방52례환자（71.27％）적시력득도료제승，Ⅳ기、Ⅴ기、Ⅵ기환자술후적시력증가솔위100.00％、90.48％화61.22％；술후환자일차성시망막복위성공솔위100％。술후파리체출혈1안，술후후발성백내장8안，전충규유적환자유7안출현고안압，3안출현규유유화。결론：파리체절제술연합백내장절제술가유효지대PDR진행치료，개선환자적시력화감소재차백내장절제술적궤솔。
Objective:To evaluate the outcome of vitrectomy combined with cataract surgery for treating proliferative diabetic retinopathy ( PDR ) .Methods:68 cease ( 74 eyes ) of PDR patients who underwent vitrectomy combined with cataract surgery from Jun.2008 to Jun.2010 were retrospectively analyzed in surgery procedure , preoperative and postoperative corrected visual acuity and complication . Results:Silicon oil injection rate in stage Ⅵ were significantly higher than that in stage Ⅳ and Ⅴ ( P<0.01 ) .Follow-up 52 cases ,71.27%showed visual acuity improved postoperatively .The postoperative visual acuity improvement rate were 100%in stage Ⅳ,90.48% in stage Ⅴ and 61.22 in stage Ⅵ and there was no significant difference among different stages ( P>0.05 ) .All eyes showed anatomic repair of retinal detachment .vitreous hemorrhage was found in 1 eye,cataract was found in 8 eyes after combined operation ,high intraocular pressure was found in 7 eyes,silicon oil emulsification was found in 3 eyes in silicon oil injection group .Conclusion:Vitrectomy combined with cataract surgery are safe and effective in treating patients with PDR and may avoid a second operation for patients with cataract and provide earlier visual rehabilitation .
