目的：运用彩色多普勒超声经腹部检测早期妊娠子宫动脉血流图血流参数和频谱特征，建立11～14孕周双侧子宫动脉血流参数，搏动指数（PI）的正常值范围。方法2013年10月1日至2014年7月1日于吉林大学第二临床医院建档行产前检查的孕妇共207例，通过彩色多普勒超声在孕11～14周检测子宫动脉血流 PI 值，记录舒张早期切迹。结果①11～14孕周子宫动脉血流 PI 值呈正态分布， PI 值正常参考值范围（95％的界限）PI（1．48±1．10）。②在11～14孕周，78．7％的孕妇存在子宫动脉切迹，其中双侧子宫动脉切迹约54．6％，单侧子宫动脉切迹24．1％。③双侧子宫动脉手动描迹和机测获得的 PI 值有统计学差异（P＜0．05）。结论早期妊娠子宫动脉血流 PI 值随着妊娠天数的增加逐渐降低。本研究建立经腹超声11～14孕周血流 PI 值参考值区间并记录舒张早期切迹特征，有助于进一步研究早期妊娠异常子宫动脉血流图与妊娠期高血压疾病、胎儿宫内生长受限等不良妊娠结局的相关性。
목적：운용채색다보륵초성경복부검측조기임신자궁동맥혈류도혈류삼수화빈보특정，건립11～14잉주쌍측자궁동맥혈류삼수，박동지수（PI）적정상치범위。방법2013년10월1일지2014년7월1일우길림대학제이림상의원건당행산전검사적잉부공207례，통과채색다보륵초성재잉11～14주검측자궁동맥혈류 PI 치，기록서장조기절적。결과①11～14잉주자궁동맥혈류 PI 치정정태분포， PI 치정상삼고치범위（95％적계한）PI（1．48±1．10）。②재11～14잉주，78．7％적잉부존재자궁동맥절적，기중쌍측자궁동맥절적약54．6％，단측자궁동맥절적24．1％。③쌍측자궁동맥수동묘적화궤측획득적 PI 치유통계학차이（P＜0．05）。결론조기임신자궁동맥혈류 PI 치수착임신천수적증가축점강저。본연구건립경복초성11～14잉주혈류 PI 치삼고치구간병기록서장조기절적특정，유조우진일보연구조기임신이상자궁동맥혈류도여임신기고혈압질병、태인궁내생장수한등불량임신결국적상관성。
Objective To detect related parameters of uterine artery blood flow and characteristics of Doppler in the first trimester of pregnancy,and to establish normal reference value range of bilateral uter-ine artery pulsatility index (PI).Method From October1 ,2013 to July1 ,2014,a total of 207pregnancies, refered to our institution for routine prenatal examination,were included in this cohort study.The left and right uterine arteries were examined by color and pulsed Doppler and the mean pulse index (PI)as well as the presence of a bilateral parly diastolic notch were recorded.Results ① Mean uterine artery PI in first tri-mester of pregnancy showed a normal distribution,the reference value range of PI is PI(1.48±1.10);② In the first trimester of pregnant,78.7% of pregnant women have uterine artery notch,54.6% is bilateral,24.1% is single;③ The difference between the manual measurement of uterine artery and mechanical measurement has statistically significance (P<0.05).Conclusions Mean uterine artery PI in the first trimester of pregnancy shows a significant linear decrease.To establish the normal reference value range of PI and the characteris-tics of early diastolic notch is helpful to further study the relevance between abnormal uterine artery blood flow and hypertensive disorders complicating pregnancy,FGR and poor pregnancy finale.
