目的:研究一种自动脱落内支架吻合技术对小口径胆肠吻合的影响。方法:16只小型猪被随机分成两组:一组常规行间断缝合胆管空肠吻合术(n=8;对照组),另一组行间断缝合加自动脱落内支架胆管空肠吻合术(n=8;实验组)。结果:吻合时间实验组(19.3±2.1)分钟,而对照组(25.4±1.8)分钟,实验组明显少于对照组(P<0.05);术后6个月对照组胆肠吻合口内径明显小于实验组[(3.0±0.3) mm vs (3.8±0.3);P<0.05];所有内支架管都在51天内自行脱落排出;除对照组发生的1例吻合口狭窄外,其余15例(对照组7例,实验组8例)肝功能、肝组织形态均正常。结论:新的内支架吻合技术对于胆管不扩张的胆肠吻合安全易行。

Objective Investigate a novel technique with a self-shed intra-anastomotic biliary stent for small-caliber hepaticojejunal anastomosis ( HJ) . Method Sixteen minipigs with small-caliber bile ducts were divided randomly in-to two groups. One group performed HJ conventionally with interrupted sutures without any stent ( n=8;control group) . One group performed HJ using a self-shed biliary stent ( n=8;experimental group) . Result The mean an-astomosis time of the experimental group was(19. 3±2. 1)minutes and that of the control group was(25. 4±1. 8)mi-nutes, that of the experimental group being significantly shorter (P<0. 05). At 6 months, the HJ caliber was smal-ler in the control group(3. 0±0. 3) mm than in the experimental group[(3. 8±0. 3)mm;P<0. 05]. All stents were discharged within 51 days. Except 1 anastomotic stricture occurred in the control group. The remaining animals (7/8 in control group, and 8/8 in experimental group) had normal liver histology and function. Conclusion The novel HJ technique is easy and safe to perform in nondilated bile ducts.
