目的：探讨早期系统化护理在伤锥置钉治疗胸腰椎骨折中的应用效果。方法：将58例行伤锥置钉治疗的胸腰椎骨折患者随机分为观察组和对照组各29例，对照组术后行常规骨科护理，观察组术后在对照组基础上行早期系统化护理。比较两组患者护理前后 VAS 评分、伤椎后凸 Cobb 角、功能障碍指数评分（ODI）、并发症发生率、住院时间及满意度评分等。结果：与护理前相比，两组护理后 VAS、ODI 评分均明显下降，观察组护理后 VAS、ODI 评分明显低于对照组护理后（P ﹤0.05）；观察组术后并发症发生率明显低于对照组（P ﹤0.05）；观察组卧床时间、住院时间明显短于对照组，满意度评分明显高于对照组（P ﹤0.05）。结论：对胸腰椎骨折行伤锥置钉治疗的患者应用早期系统化护理干预能有效改善患者胸腰椎功能，缩短了患者住院时间，有利于患者康复。
목적：탐토조기계통화호리재상추치정치료흉요추골절중적응용효과。방법：장58례행상추치정치료적흉요추골절환자수궤분위관찰조화대조조각29례，대조조술후행상규골과호리，관찰조술후재대조조기출상행조기계통화호리。비교량조환자호리전후 VAS 평분、상추후철 Cobb 각、공능장애지수평분（ODI）、병발증발생솔、주원시간급만의도평분등。결과：여호리전상비，량조호리후 VAS、ODI 평분균명현하강，관찰조호리후 VAS、ODI 평분명현저우대조조호리후（P ﹤0.05）；관찰조술후병발증발생솔명현저우대조조（P ﹤0.05）；관찰조와상시간、주원시간명현단우대조조，만의도평분명현고우대조조（P ﹤0.05）。결론：대흉요추골절행상추치정치료적환자응용조기계통화호리간예능유효개선환자흉요추공능，축단료환자주원시간，유리우환자강복。
Objective:To investigate the effect of early systematic nursing care in the treatment of thoracolumbar spinal fracture by trans-pedicular fixation via injured vertebra. Methods:58 thoracolumbar spinal fracture patients who were treated with transpedicular fixation via injured vertebra were randomly divided into the observation group and the control group(29 cases in each group). The conventional post-operative nursing care for fracture patients was given to the patients in the control group;the early systematic nursing care was addtionally provided for the patients in the observation group. The VAS score,the Cobb angle of the injured vertebra,ODI score,the incidence of com-plicatios,hospitalization days and the score of satisfaction rate were compared between the two groups before and after nursing care. Re-sults:Compared with those before nursing care,the scores of VAS and ODI significantly decreased after nursing care in the two groups and the scores of VAS and ODI were significantly lower in the observation group than the control group after nursing care(P ﹤ 0. 05);the inci-dence of postoperative complications was significantly lower in the observation group than the control group(P ﹤ 0. 05);the time in bed and the time of hospitalization was significantly shorter and the score of satisfaction rate was significantly higher in the observation group than the control group(P ﹤ 0. 05). Conclusion:The early systematic nursing intervention can effectively improve thoracic and lumbar func-tion of the patients and shorten the hospitalization time in the treatment of thoracolumbar spinal fracture by transpedicular fixation via in-jured vertebra.
