目的:探讨血小板的变化对有机磷中毒患者预后的影响。方法:将167例有机磷中毒患者按血小板(PLT)是否减少分为PLT减少组和PLT非减少组,按随访结果再分为存活组和死亡组,所有患者入院后行中毒程度评估;记录血小板在1、3、5、7、10 d血小板计数,观察多器官功能障碍(MODS)发生率。结果:PLT减少组住院死亡率及多器官功能衰竭发生率明显高于PLT非减少组(P<0.05),但有机磷中毒初期病情程度分布无差异性(P>0.05);死亡组与生存组相比血小板计数多在第5天后进行性下降(P<0.05);结论:有机磷中毒患者血小板的进行性下降能在一定程度上反应患者预后不良。

목적:탐토혈소판적변화대유궤린중독환자예후적영향。방법:장167례유궤린중독환자안혈소판(PLT)시부감소분위PLT감소조화PLT비감소조,안수방결과재분위존활조화사망조,소유환자입원후행중독정도평고;기록혈소판재1、3、5、7、10 d혈소판계수,관찰다기관공능장애(MODS)발생솔。결과:PLT감소조주원사망솔급다기관공능쇠갈발생솔명현고우PLT비감소조(P<0.05),단유궤린중독초기병정정도분포무차이성(P>0.05);사망조여생존조상비혈소판계수다재제5천후진행성하강(P<0.05);결론:유궤린중독환자혈소판적진행성하강능재일정정도상반응환자예후불량。
Objective:To study the effects of the platelet changes to evaluate prognosis in acute organophosphorus pesticide poisoning(AOPP). Method:167 AOPP patients who were divided into thrombocytopenic group and non-thrombocytopenic group according to the number of the platelet, and into survivor group and non- survivor group according to the results of following up,then,recorded the incidence rate of MODS,the hospital mortality,platelet counts after entering intensive care unit 1,3,5,7,10 days,and assessed the severity of classification of organic phosphorus poisoning. Result:The incidence rate of MODS and the hospital mortality of thrombocytopenic patients were higher than those in non thrombocytopenic patients(P<0.05). There was no significant difference in the severity of disease classification between thrombocytopenic group and non-thrombocytopenic group(P>0.05). The platelet count of non-surviors group on the 5 days after entered ICU began to decline than in survivors(P<0.05). Conclusion The changes of refractory thrombocytopenia are sensitive of the state of patient and prognosis of patients with acute organophosphorus pesticide poisoning.
