目的 分析无创正压通气用于治疗心肌梗死合并肺水肿的临床疗效.方法 选取70例急性心肌梗死合并肺水肿的患者,随机分成两组,对照组35例使用普通面罩吸氧等常规治疗;观察组35例在常规治疗基础上进行双向正压通气治疗.观察患者的心率、血压、呼吸频率、血清BNP以及动脉血气指标的变化.结果 观察组进行正压通气治疗2h和24 h后呼吸频率、PaO2改善情况均好于对照组,差异具有统计学意义(P<0.05);治疗72 h后,治疗组的血清BNP水平明显低于对照组(P<0.01).结论 无创正压通气能够有效改善急性心肌梗死患者的肺水肿,具有较好的疗效,适合推广应用.

목적 분석무창정압통기용우치료심기경사합병폐수종적림상료효.방법 선취70례급성심기경사합병폐수종적환자,수궤분성량조,대조조35례사용보통면조흡양등상규치료;관찰조35례재상규치료기출상진행쌍향정압통기치료.관찰환자적심솔、혈압、호흡빈솔、혈청BNP이급동맥혈기지표적변화.결과 관찰조진행정압통기치료2h화24 h후호흡빈솔、PaO2개선정황균호우대조조,차이구유통계학의의(P<0.05);치료72 h후,치료조적혈청BNP수평명현저우대조조(P<0.01).결론 무창정압통기능구유효개선급성심기경사환자적폐수종,구유교호적료효,괄합추엄응용.
Objective To analyze the clinical efficacy of noninvasive positive pressure ventilation (NIPPV) for treatment of acute myocardial infarction (AMI) with pulmonary edema.Methods 70 patients with AMI combined with pulmonary edema,were selected and randomly divided into two groups,the control group with 35 patients used ordinary oxygen mask conventional treatment; observation group with 35 patients was given conventional treatment on the basis of a two-way positive airway pressure therapy.Then we observed the patients' heart rate,blood pressure,respiratory rate,serum BNP and arterial blood gas index changes.Result The observation group was positive airway pressure therapy 2 h and 24 h,respiratory rate,PaO2 improving situation better than that of the control group,the difference was statistically significant (P <0.05); 72 h after treatment,the observation group,serum BNP levels were significantly lower than those of the control group (P<0.01).Conclusion NIPPV can be effective in improving pulmonary edema in patients with AMI,with good effect,which is suitable for promotion and application.
