目的评价半月板损伤与对应软骨T2值的关系。方法回顾性分析90例(168个半月板)行膝关节常规MRI及T2-mapping序列检查的骨关节炎(osteoarthritis,OA)患者。病例按其后角半月板损伤程度分组,0级为对照组(15例),Ⅰ、Ⅱ、Ⅲ级为病例组(65例)。应用MRI T2-mapping序列测量各组对应于半月板后角的股骨内、外侧髁及胫骨内、外侧髁关节软骨的T2值,探讨半月板损伤与软骨T2值的关系。结果Ⅰ、Ⅱ级与对照组关节软骨T2值差异无统计学意义(P=0.53、0.99)。Ⅲ级关节软骨T2值高于Ⅰ、Ⅱ级组(P<0.05)和对照组(P<0.01)。61.7%的Ⅲ级信号见于内侧半月板后角(29/47)。Ⅲ级组胫骨、股骨内侧髁软骨T2值高于胫骨、股骨外侧髁软骨(P<0.05)。结论半月板Ⅲ级损伤时对应处的关节软骨T2值明显升高,T2-mapping成像对诊断软骨损伤有很高的临床应用价值。

목적평개반월판손상여대응연골T2치적관계。방법회고성분석90례(168개반월판)행슬관절상규MRI급T2-mapping서렬검사적골관절염(osteoarthritis,OA)환자。병례안기후각반월판손상정도분조,0급위대조조(15례),Ⅰ、Ⅱ、Ⅲ급위병례조(65례)。응용MRI T2-mapping서렬측량각조대응우반월판후각적고골내、외측과급경골내、외측과관절연골적T2치,탐토반월판손상여연골T2치적관계。결과Ⅰ、Ⅱ급여대조조관절연골T2치차이무통계학의의(P=0.53、0.99)。Ⅲ급관절연골T2치고우Ⅰ、Ⅱ급조(P<0.05)화대조조(P<0.01)。61.7%적Ⅲ급신호견우내측반월판후각(29/47)。Ⅲ급조경골、고골내측과연골T2치고우경골、고골외측과연골(P<0.05)。결론반월판Ⅲ급손상시대응처적관절연골T2치명현승고,T2-mapping성상대진단연골손상유흔고적림상응용개치。
Objective To assess the relation between severity of meniscus injury and T2 value of knee joint cartilages. Methods Ninety osteoarthritis (OA) patients (168 menisci) who underwent conventional MRI and T2-mapping were retrospectively analyzed. The patients were divided into control group (n=15) with grade 0 meniscus injury and patients group (n=65) with gradesⅠ,Ⅱ,Ⅲmeniscus injury. T2 values of medial femoral and lateral condyle cartilages, and medial tibial and lateral condyle cartilages adjacent to the center of meniscus posterior horn were measured. Relation between severity of meniscus injury and T2 values of knee joint cartilages was studied. Results No significant difference was found in T2 values of knee joint cartilages between patients with grade Ⅰ / Ⅱmeniscus injury and controls (P=0. 53, P=0.99). The T2 values of knee joint cartilages were significantly higher in patients with gradeⅢmeniscus injury than in controls and in patients with gradeⅠ/Ⅱmeniscus injury (P<0.05). Signals were observed in medial meniscus posterior horns of 61.7%patients (29/47) with gradeⅢmeniscus injury. The T2 values of medial tibial and femoral cartilages were significantly higher than those of lateral tibial and femoral cartilages in patients with gradeⅢmeniscus injury (P< 0.05). Conclusion The T2 values of knee joint cartilages are significantly higher in patients with grade Ⅲ meniscus injury. T2-mapping plays an important role in diagnosis of knee joint cartilage injury.
