目的:探讨基质金属蛋白酶-2(MMP-2)和血管内皮生长因子(VEGF)在子宫内膜癌组织中的表达及其与临床病理特征间的关系。方法:采用免疫组化技术检测96例子宫内膜癌,56例子宫内膜增生症,20例正常子宫内膜中MMP-2和VEGF的表达,并分析其与子宫内膜癌的临床分期、组织学分级、肌层深度及淋巴结转移等病理参数的关系。结果:①MMP-2和VEGF在正常子宫内膜,子宫内膜增生症,I-II期和II-IV期子宫内中的表达逐渐增强,在子宫内膜增生症、I-II期和III-IV期子宫内膜癌之间,二者表达差异有显著性,p<0.05;但在正常子宫内膜和子宫内膜增生症之间差异无统计学意义,p>0.05。②VEGF和MMP-2在III-IV期子宫内膜癌中的阳性率分别为75%和70%;VEGF的表达与子宫内膜癌手术—病理分期、肌层浸润程度、淋巴结转移具有相关性,p<0.05;与年龄、组织学分级无关,p>0.05。在淋巴结转移阳性率和阴性率中分别为80%和42.9%,二者差异具有统计学意义,p<0.05;在III-IV期子宫内膜癌组表达率为75%,明显高于I-II期组的46.4%,p<0.05;③MMP-2的表达与年龄无关,p>0.05,但在子宫内膜组织学分、手术—病理分期、肌层润程度、淋巴结转移均相关、各组中的表达差异均有显著性,p<0.05。结论:MMP-2和 VEGF 在子宫内膜癌的发生和发展中起到了重要作用,共同检测对指导临床治疗和判断,预后有重要作用。

목적:탐토기질금속단백매-2(MMP-2)화혈관내피생장인자(VEGF)재자궁내막암조직중적표체급기여림상병리특정간적관계。방법:채용면역조화기술검측96례자궁내막암,56례자궁내막증생증,20례정상자궁내막중MMP-2화VEGF적표체,병분석기여자궁내막암적림상분기、조직학분급、기층심도급림파결전이등병리삼수적관계。결과:①MMP-2화VEGF재정상자궁내막,자궁내막증생증,I-II기화II-IV기자궁내중적표체축점증강,재자궁내막증생증、I-II기화III-IV기자궁내막암지간,이자표체차이유현저성,p<0.05;단재정상자궁내막화자궁내막증생증지간차이무통계학의의,p>0.05。②VEGF화MMP-2재III-IV기자궁내막암중적양성솔분별위75%화70%;VEGF적표체여자궁내막암수술—병리분기、기층침윤정도、림파결전이구유상관성,p<0.05;여년령、조직학분급무관,p>0.05。재림파결전이양성솔화음성솔중분별위80%화42.9%,이자차이구유통계학의의,p<0.05;재III-IV기자궁내막암조표체솔위75%,명현고우I-II기조적46.4%,p<0.05;③MMP-2적표체여년령무관,p>0.05,단재자궁내막조직학분、수술—병리분기、기층윤정도、림파결전이균상관、각조중적표체차이균유현저성,p<0.05。결론:MMP-2화 VEGF 재자궁내막암적발생화발전중기도료중요작용,공동검측대지도림상치료화판단,예후유중요작용。
Objective:To study of MMP-2 and VEGF in endometrial carcinoma tissue of the express and the relationship between clinical and pathological features. Methods:Immunohistochemical technical detection 96 example palace film carcinoma, 56 examples palace film hyperplasia disease, twenty cases of normal endometrium of MMP-2 and VEGF expression, and analyzes its and endometrial cancer clinical stage, histological grade, muscle layer depth and lymph node metastasis and the relationship between pathological parameters. Results: (1) The MMP - 2 and VEGF in normal endometrium, endometrial hyperplasia disease, I - phase II and II - stage IV uterus strengthen gradual y, the expression in endometrial hyperplasia disease, I-phase II and III-stage IV endometrial carcinoma, the expression differences between significant, p<0.05;But in normal endometrium and endometrial hyperplasia disease no statistical y significant differences between, p>0.05. (2) The VEGF and MMP-2 in III-stage IV endometrial carcinoma of the positive rate were 75% and 70% respectively; The expression of VEGF and endometrial cancer surgery - pathologic stage, muscle layer infiltration degree, lymph node metastasis has relevance, p<0.05;Has nothing to do with age, histologic grading, p>0.05. In the positive and negative lymph node metastasis in rate were 80%and 42.9%respectively, the difference is statistical y significant, p<0.05;In III-stage IV endometrial carcinoma group expression rate is 75%, much higher than I-phase II group of 46.4%, p<0.05;(3) The expression of MMP-2 and regardless of age, p>0.05, but in endometrial tissue credits, operation-pathologic stage, muscle layer embel ish degree, lymph node metastasis are related, the expression of differences are significant, p<0.05. Conclusion:MMP-2 and VEGF in endometrial cancer occurrence and development plays an important role in guiding the common testing clinical treatment and prognosis judgement, has an important role.
