目的观察冠心病伴牙周炎患者在牙周治疗前后血清sICAM-1的变化。方法选取冠心病(稳定期)伴牙周炎患者55例,分为牙周治疗+心内科治疗组29例(实验组)和心内科治疗组26例(对照组)。对实验组患者行牙周治疗,采集2组治疗前和治疗后1个月牙周临床参数和血清样本。应用酶联免疫吸附法测定血清sICAM-1浓度。结果实验组和对照组患者治疗前牙周临床参数和血清sICAM-1浓度无统计学差异(P>0.05);实验组患者经过牙周治疗后的牙周临床参数和血清sICAM-1浓度较治疗前均明显降低,差异有统计学意义(P<0.05)。而对照组患者各项指标无明显变化。结论冠心病伴牙周炎患者通过牙周治疗可以降低血清中sICAM-1浓度,改善血管内皮细胞功能,利于冠心病的预防和治疗,降低其再发风险。

Objective To investigate the change of soluble intracellular adhesion molecule-1(sICAM-1) on serum in coronary heart disease patients with chronic periodontitis after periodontal therapy.Methods 55 patients with coronary heart disease and chronic periodontitis were selected.Sulcus bleeding index(SBI),probing index(PD),clinical attachment level(AL),sICAM-1 were tested respectively in ifrst visit and review.29 of total were treated by periodontal therapy.Results 29 patients demonstrated signiifcant improvement in clinical periodontal index in PD,AL and SBI.The concentration of sICAM-1 on serum was decreased.The rest of the 26 patients did not change in clinical periodontal index and serum index.Conclusion Periodontal treatment can reduce serum concentration of sICAM-1,improve endothelial cell function,beneifcial to the prevention and treatment of coronary heart disease and reduce the risk of recurrence.
