目的：对老年卒中相关性肺炎的危险因素进行分析，探讨其临床干预措施。方法采用病例对照的研究方法，纳入住院的老年卒中相关性肺炎组患者178例，卒中未合并肺炎组患者150例，对两组患者的一般资料（年龄、性别、既往病史、吸烟史、营养状况），卒中类型、严重程度（意识水平、吞咽障碍、肢体反应）、生化指标等进行单因素分析，相关结果再采用多因素 Logistic回归分析卒中相关性肺炎的危险因素。结果多因素 Logistic回归分析显示年龄（OR＝2．125），美国国立卫生院脑卒中量表（NIHSS）评分（OR ＝1．865），格拉斯哥昏迷评分（Glasgow Coma Scale，GCS）（OR＝1．416），吞咽困难（OR＝1．686），完全卧床（OR ＝2．008），合并高血糖（OR ＝1．123），低蛋白血症（OR＝1．452），高皮质醇水平（OR＝1．033），是老年卒中相关性肺炎发病的危险因素。结论老年卒中相关性肺炎的发生、发展存在多种高危因素，需要早期发现和预防。
목적：대노년졸중상관성폐염적위험인소진행분석，탐토기림상간예조시。방법채용병례대조적연구방법，납입주원적노년졸중상관성폐염조환자178례，졸중미합병폐염조환자150례，대량조환자적일반자료（년령、성별、기왕병사、흡연사、영양상황），졸중류형、엄중정도（의식수평、탄인장애、지체반응）、생화지표등진행단인소분석，상관결과재채용다인소 Logistic회귀분석졸중상관성폐염적위험인소。결과다인소 Logistic회귀분석현시년령（OR＝2．125），미국국립위생원뇌졸중량표（NIHSS）평분（OR ＝1．865），격랍사가혼미평분（Glasgow Coma Scale，GCS）（OR＝1．416），탄인곤난（OR＝1．686），완전와상（OR ＝2．008），합병고혈당（OR ＝1．123），저단백혈증（OR＝1．452），고피질순수평（OR＝1．033），시노년졸중상관성폐염발병적위험인소。결론노년졸중상관성폐염적발생、발전존재다충고위인소，수요조기발현화예방。
Objective To analyze the risk factors for senile stroke-associated pneumonia(SAP)and to explore the clinical intervention measures.Methods A case-control study was conducted in 178 hospitalized senile patients who were diagnosed SAP as patient group,and 150 patients who were diagnosed stroke without pneumonia as control group. General clinical data (age,gender,history),category and severity of stroke(dysphasia,disturbance of consciousness and motor response)as well as blood tests were compared between two groups.Multifactor logistic regression was used to screen the associated factors.Results Age (OR =2.125),totally bedridden(OR =2.008),NIHSS score(OR =1.865),GCS score (OR =1.416),dysphasia(OR =1.686),hyperglycemia(OR =1.123),hypoalbuminemi(OR =1.452),high level of cortisol(OR =1.033)were related with stroke associated pneumonia.Conclusion Senile SAP involves in multiple factors and needs prevention in advance.
