目的:探讨类风湿关节炎合并肺间质病变(RA-ILD)患者的临床血清中白介素-33(IL-33)表达,特征外周血单个核细胞(PBMC)。方法类风湿关节炎(RA)患者115例,分为RA-ILD组(29例)和RA组(86例),另选取健康自愿者20例为对照组,类风湿因子(RF)、抗环瓜氨酸抗体(anti-CCP)及抗角蛋白抗体( AKA)的浓度,检测PBMC中IL-33 mRNA表达,同时通过酶联免疫吸附法( ELISA)测定各组血清中IL-33的表达。结果与RA患者比,RA-ILD组除性别外平均年龄、病程比较差异均有统计学意义,RF、anti-CCP及AKA的浓度在RA-ILD与RA两组之间无统计学差异(P>0.05),RA-ILD组PBMC中IL-33 mRNA及血清中IL-33表达含量明显比RA及正常对照组升高( P<0.01)。结论 RA-ILD易发生于年龄大、病程较长的RA患者,一般实验检查不能预测RA患者ILD发生的可能性;PBMC和血清中IL-33可能与RA-ILD的发病有关;IL-33水平增高可能为RA-ILD的预测指标。

목적:탐토류풍습관절염합병폐간질병변(RA-ILD)환자적림상혈청중백개소-33(IL-33)표체,특정외주혈단개핵세포(PBMC)。방법류풍습관절염(RA)환자115례,분위RA-ILD조(29례)화RA조(86례),령선취건강자원자20례위대조조,류풍습인자(RF)、항배과안산항체(anti-CCP)급항각단백항체( AKA)적농도,검측PBMC중IL-33 mRNA표체,동시통과매련면역흡부법( ELISA)측정각조혈청중IL-33적표체。결과여RA환자비,RA-ILD조제성별외평균년령、병정비교차이균유통계학의의,RF、anti-CCP급AKA적농도재RA-ILD여RA량조지간무통계학차이(P>0.05),RA-ILD조PBMC중IL-33 mRNA급혈청중IL-33표체함량명현비RA급정상대조조승고( P<0.01)。결론 RA-ILD역발생우년령대、병정교장적RA환자,일반실험검사불능예측RA환자ILD발생적가능성;PBMC화혈청중IL-33가능여RA-ILD적발병유관;IL-33수평증고가능위RA-ILD적예측지표。
Objective To study the expression of IL-33 in patients with rheumatoid arthritis complicated with interstitial lung disease. Methods 115 rheumatoid arthritis ( RA) patients were divided into the RA-ILD group (29 cases) and the RA group (86 cases), and 20 healthy people were selected as the control group. The concentra-tion of RF, anti-CCP and AKA was detected in each group. Meanwhile, the expression of IL-33mRNA in peripheral blood mononuclear cells ( PBMC) was detected by RT-PCR and the expression of IL-33 in serum was detected by en-zyme-linked immunosorbent assay ( ELISA) . Results There were significant differences in mean age and course of disease between the RA-ILD group and the RA group, except gender. The concentration of RF, anti-CCP and AKA had no significant difference between the two groups (P>0. 05). However, the expression of IL-33 mRNA in PBMC and serum level of IL-33 in the RA-ILD group were significantly higher than those in the RA group and the control group (P<0. 01). Conclusion RA-ILD easily occurs in elderly RA patients with longer course of disease. The general clinical laboratory tests can not predict the incidence of RA patients with ILD. IL-33 in PBMC and serum may be related to RA-ILD, and the high expression of IL-33 can be a predictor of RA-ILD.
