目的:探讨脑肠肽Ghrelin对内毒素(LPS)诱导急性肺损伤(ALI)大鼠肺血管通透性的影响。方法 SD大鼠随机分为生理盐水对照组、ALI模型组及Ghrelin治疗组,每组12只,静脉注射LPS(5 mg/kg)制备ALI模型,Ghrelin治疗组在LPS注射30 min后经腹腔给予Ghrelin (40 nmol/kg),对照组给予等体积生理盐水。观察6 h后处死动物,取肺组织计算肺湿/干(W/D)及行病理学检查,肺组织匀浆检测伊文思蓝( EB)提取含量和肺组织髓过氧化物( MPO)活性,收集肺泡灌洗液( BALF)检测其中炎性细胞总数、百分比及蛋白含量,取动脉血行血气分析。结果与ALI模型组比较,Ghrelin治疗组肺W/D、EB提取含量及MPO活性均明显降低( P<0.05), BALF中炎性细胞总数及蛋白含量亦明显降低( P<0.05)。血气结果比较,Ghrelin治疗组较ALI模型组PaO2明显上升(P<0.01),PaCO2明显下降(P<0.05),pH值明显升高(P<0.05)。病理检测提示,Ghrelin治疗组肺组织损伤程度较ALI模型组明显减轻。结论 Ghrelin可降低ALI肺血管通透性,对LPS诱导的ALI大鼠发挥保护作用。

목적:탐토뇌장태Ghrelin대내독소(LPS)유도급성폐손상(ALI)대서폐혈관통투성적영향。방법 SD대서수궤분위생리염수대조조、ALI모형조급Ghrelin치료조,매조12지,정맥주사LPS(5 mg/kg)제비ALI모형,Ghrelin치료조재LPS주사30 min후경복강급여Ghrelin (40 nmol/kg),대조조급여등체적생리염수。관찰6 h후처사동물,취폐조직계산폐습/간(W/D)급행병이학검사,폐조직균장검측이문사람( EB)제취함량화폐조직수과양화물( MPO)활성,수집폐포관세액( BALF)검측기중염성세포총수、백분비급단백함량,취동맥혈행혈기분석。결과여ALI모형조비교,Ghrelin치료조폐W/D、EB제취함량급MPO활성균명현강저( P<0.05), BALF중염성세포총수급단백함량역명현강저( P<0.05)。혈기결과비교,Ghrelin치료조교ALI모형조PaO2명현상승(P<0.01),PaCO2명현하강(P<0.05),pH치명현승고(P<0.05)。병리검측제시,Ghrelin치료조폐조직손상정도교ALI모형조명현감경。결론 Ghrelin가강저ALI폐혈관통투성,대LPS유도적ALI대서발휘보호작용。
Objective To observe the effects of ghrelin on pulmonary capillary permeability of acute lung injury ( ALI) caused by LPS.Methods Male SD rats were randomly divided into three groups:saline control group which received normal saline , ALI group which received LPS (5 mg/kg) to induce ALI model, Ghrelin therapy group were intraperitoneally injected with ghrelin (40 nmol/kg) 30 min after LPS challenge.Rats were sacrificed at 6 h after LPS injection.Arterial blood was collected for determination blood gas . Lung tissue was removed and the right lung was lavaged to collected bronchoalveolar lavage fluid ( BALF) for determination of protein content , and differential inflammatory cell counts . The left lung was used for microscopic examination , wet/dry lung weight ratio , myeloperoxidase (MPO) activity and Evans Blue (EB) content.Results In ghrelin therapy group, lung weight ratio , EB content and W/D ratio in lung tissue were significantly reduced compared with those in ALI group (P <0.05).The total and differential inflammatory cells were also significantly reduced in ghrelin therapy group which compared with ALI group .The level of PaO 2 of ghrelin therapy group was increased significantly compared with ALI group (P<0.01), the level of PaCO2 decreased significantly compared with ALI group ( P <0.05 ), and the value of pH was significantly higher compared with ALI group (P<0.05).Pathological examination showed that the lung injuries of ghrelin therapy group were significantly reduced compared with those of ALI group .Conclusion Ghrelin shows a marked reductive effect on the capillary permeability , reduce the injury of lung tissue and exert protective effects on lung in ALI rats .
