目的：评价原位尿道板加游离包皮内板尿道成形术I期治疗重型尿道下裂的临床疗效。方法回顾分析我们2006年1月-2014年1月间采用原位尿道板加游离包皮内板尿道成形术I期治疗重型尿道下裂82例的临床资料并进行满意度调查。结果随访1年，73例一次手术成功，总成功率89.02%；7例（8.54%）出现尿道瘘，经再次修补均已治愈；2例（2.44%）出现尿道外口狭窄,经尿道扩张后治愈。术后患者满意度为81.71%（67/82）；医师满意度为89.02%（73/82）。结论原位尿道板加游离包皮内板尿道成形术治疗重型尿道下裂具有疗效显著，患者满意度较高，可作为I期手术治疗的首选。

Objective To evaluate clinical efficacy and patient satisfaction of urethroplasty repair via combing urethral plate with free inner preputial skin plat in stage-I treatment of severe hypospadias. Methods One-year folow-up survey of satisfaction rate by 82 external genitalia patients who had received surgery during the years from January, 2006 to January, 2014. Al the patients were diagnosed as of severe hypospadias and treated with urethroplasty repair by using both urethral plate and free inner preputial skin plate. Results The survey of 82 patients shows: 73/82 (89.02%) patients were cured successfuly after Stage-I repair treatment. Urethr-ocutaneous fistula occurred in 7 patients (8.54%), and a second therapy were performed; mild meatal stenosis occurred in 2 patients (2.44% ) and were treated by urethral sounding; and then al the 9 patients were cured successfuly. The rate of satisfaction was 81.71% (67/82) by patients and 89.02% (73/82) by doctors. Conclusion The survey proved that combination of urethral plate and free inner preputial skin plate in urethroplasty repair show positive efficacy and outstanding patient satisfaction. The treatment could be recommended as the first choice for Stage-I treatment and repair of severe hypospadias.
