目的:探讨血清内脂素水平与多囊卵巢综合征(PCOS)患者胰岛素抵抗(IR)及肥胖的相关性。方法选择 PCOS 患者23例,对照组15例,根据 BMI 将 PCOS 组分为 PCOS 肥胖组8例(BMI≥25)和 PCOS 非肥胖组15例(BMI <25);对照组分为对照肥胖组7例和对照非肥胖组8例。采用 ELISA 检测各组空腹血清内脂素浓度,同时于早卵泡期检测性激素水平、空腹血糖、空腹胰岛素(FINS),计算 BMI、腰臀比、胰岛素抵抗指数(HOMA-IR)。结果PCOS 患者血清内脂素水平高于对照组(P <0.05)。PCOS 肥胖组血清内脂素水平高于对照肥胖组(P <0.05),而 PCOS 非肥胖组与对照非肥胖组血清内脂素水平比较差异无统计学意义(P >0.05)。相关性分析显示,血清内脂素水平与 BMI、睾酮、空腹血糖、FINS、HOMA-IR 呈正相关(P 均<0.05)。结论PCOS 患者存在血清内脂素的代谢异常,其血清内脂素水平的升高可能与肥胖、胰岛素抵抗有关;血清内脂素水平可作为 PCOS 患者 IR 程度的预测指标之一。

목적:탐토혈청내지소수평여다낭란소종합정(PCOS)환자이도소저항(IR)급비반적상관성。방법선택 PCOS 환자23례,대조조15례,근거 BMI 장 PCOS 조분위 PCOS 비반조8례(BMI≥25)화 PCOS 비비반조15례(BMI <25);대조조분위대조비반조7례화대조비비반조8례。채용 ELISA 검측각조공복혈청내지소농도,동시우조란포기검측성격소수평、공복혈당、공복이도소(FINS),계산 BMI、요둔비、이도소저항지수(HOMA-IR)。결과PCOS 환자혈청내지소수평고우대조조(P <0.05)。PCOS 비반조혈청내지소수평고우대조비반조(P <0.05),이 PCOS 비비반조여대조비비반조혈청내지소수평비교차이무통계학의의(P >0.05)。상관성분석현시,혈청내지소수평여 BMI、고동、공복혈당、FINS、HOMA-IR 정정상관(P 균<0.05)。결론PCOS 환자존재혈청내지소적대사이상,기혈청내지소수평적승고가능여비반、이도소저항유관;혈청내지소수평가작위 PCOS 환자 IR 정도적예측지표지일。
Objective To explore the relationship among serum visfatin and insulin resistance (IR) and obesity in women with polycystic ovary syndrome (PCOS).Methods Twenty-three patients with PCOS were assigned into the PCOS group and 1 5 normal subjects allocated into the control group.Based upon body mass index (BMI),the PCOS group was divided into the obesity (BMI≥25,n =8)and non-obesity groups (BMI <25,n =1 5),and the control group into the obesity (n =7)and non-obesity groups (n =8).During early follicular period,sex hormone,fasting plasma glucose (FPG)and fasting insulin (FINS)were detected.Body mass index (BMI),waist to hip ratio (WHR)and homeostasis model assessment-insulin resistance (HOMA-IR)were calculated.The concentration of serum visfatin was assayed by ELISA.Results Serum visfatin in PCOS patients was significantly higher compared with that in normal controls (P <0.05).The serum level of visfatin in obese PCOS women was significantly higher than that in obese normal controls (P <0.05), whereas no statistical significance was observed in non-obese subjects between PCOS and control groups (P >0.05).Correlation analysis revealed that serum levels of visfatin were positively correlated with BMI,T,FPG, FINS and HOMA-IR (all P <0.05).Conclusions Abnormal metabolism of serum visfatin could be observed in women with PCOS.Increased serum visfatin may be correlated with obesity and IR.Serum visfatin may serve as one of the predictors for the severity of IR in PCOS women.
