目的：探讨胆道支架置入术联合三维适形放疗治疗恶性梗阻性黄疸的疗效。方法对不能手术切除的恶性阻塞性黄疸患者50例，先行胆道支架引流术，再行三维适形放射治疗。观察患者胆红素消退、支架畅通情况、近期疗效、生存时间和生存率。结果经过联合治疗后，患者黄疸症状均改善明显，血清胆红素明显下降（P＜0．01），肿瘤有效率（PR＋CR）为90．0％（45／50）。1年和2年的累积生存率分别为60．0％和11．8％，中位生存时间13个月。结论对不能手术的恶性阻塞性黄疸患者采取胆道支架置入术后联合三维适形放疗能够改善症状，提高患者生活质量，延长生存时间，是一个安全有效的治疗方法。
목적：탐토담도지가치입술연합삼유괄형방료치료악성경조성황달적료효。방법대불능수술절제적악성조새성황달환자50례，선행담도지가인류술，재행삼유괄형방사치료。관찰환자담홍소소퇴、지가창통정황、근기료효、생존시간화생존솔。결과경과연합치료후，환자황달증상균개선명현，혈청담홍소명현하강（P＜0．01），종류유효솔（PR＋CR）위90．0％（45／50）。1년화2년적루적생존솔분별위60．0％화11．8％，중위생존시간13개월。결론대불능수술적악성조새성황달환자채취담도지가치입술후연합삼유괄형방료능구개선증상，제고환자생활질량，연장생존시간，시일개안전유효적치료방법。
Objective To explore the therapeutic effect of percutaneousbiliary stent placement combined with 3-dimen-sional conformal radiotherapy for malignant biliary obstructive jaundice .Methods Percutaneous biliary stent placement combined with 3-dimensional conformal radiotherapy was performed in 50 patients with malignant biliary obstructive jaundice who were un-willing or unsuitable for surgery .Improvement of jaundice , biliary stent re-blockage , survival time and survival rates were ob-served.Results After treatment,the biliary obstruction symptoms improved markerly ,and the level of bilirubin dropped obviously (P<0.01),total response rate was 90.0%(45/50).1-and 2-year survival rates were 60.0%and 11.8%respectively.The me-dian surviving time was 13 months.Conclusion Percutaneous biliary stent placement combined with 3-dimensional conformal ra-diotherapy for malignant biliary obstructive jaundice can improve life quality and survival rate of patients ,it is a safe and effective method .
