目的：探讨胸腔逆行温热灌注卡介菌多糖治疗恶性转移性胸腔积液的临床效果。方法选择100例患者，分为2组，各50例，对照组使用顺铂逆行推注胸腔，观察组则使用卡介菌多糖核酸，逆行推注胸腔，比较两组治疗前后T细胞亚群情况，治疗后白蛋白、总蛋白变化及治疗期间发生的不良反应。结果观察组治疗后CD3、CD4和CD8比率高于治疗前和治疗后的对照组（P＜0．05），CD4／CD8比值低于治疗前和治疗后的对照组（P＜0．05），观察组出现恶心呕吐、白细胞减少及肾功能损伤比率少于对照组（P＜0．05），观察组治疗后白蛋白及总蛋白水平高于对照组（P＜0．05），观察组的临床总有效率为86．0％，对照组为40．0％，2组总有效率比较差异具有统计学意义（ P＜0．05）。结论胸腔内逆行注射温热卡介菌多糖核酸，通过高温杀灭肿瘤细胞，提高了患者免疫力，固定胸膜腔减少胸水生成。
목적：탐토흉강역행온열관주잡개균다당치료악성전이성흉강적액적림상효과。방법선택100례환자，분위2조，각50례，대조조사용순박역행추주흉강，관찰조칙사용잡개균다당핵산，역행추주흉강，비교량조치료전후T세포아군정황，치료후백단백、총단백변화급치료기간발생적불량반응。결과관찰조치료후CD3、CD4화CD8비솔고우치료전화치료후적대조조（P＜0．05），CD4／CD8비치저우치료전화치료후적대조조（P＜0．05），관찰조출현악심구토、백세포감소급신공능손상비솔소우대조조（P＜0．05），관찰조치료후백단백급총단백수평고우대조조（P＜0．05），관찰조적림상총유효솔위86．0％，대조조위40．0％，2조총유효솔비교차이구유통계학의의（ P＜0．05）。결론흉강내역행주사온열잡개균다당핵산，통과고온살멸종류세포，제고료환자면역력，고정흉막강감소흉수생성。
Objective To investigate the clinical effect of pleural retrograde hyperthermic perfusion of BCG -PSN for metastatic malignant pleural effusion .Methods 100 patients were divided into 2 groups,each with 50 patients,the control group received pleural perfusion of cisplatin ,the observation group received pleural retrograde perfusion of BCG-PSN,T cell subsets be-fore and after treatment ,albumin and total protein changes after treatment ,and adverse reactions during treatment were compared of the 2 groups.Results CD3,CD4 and CD8 ratios after treatment in the observation group were significantly higher than those of the control group before and after treatment (P<0.05),CD4/CD8 ratio was significantly lower than that of the control group be-fore and after treatment(P<0.05),nausea and vomiting,neutropenia and renal injury rate in the observation group were signifi-cantly less than those of the control group (P<0.05),after treatment,albumin and total protein levels in the observation group were significantly higher than those of the control group (P<0.05).Total effective rate in the observation group was 86.0%,to-tal effective rate in the control group was 40.0%,the difference was statistically significant of the 2 groups (P<0.05).Conclu-sion Pleural retrograde hyperthermic perfusion of BCG -PSN can kill tumor cells with high temperature ,improve patients ’ immu-nity,and reduce fixed pleural effusion formation .
