目的：探讨扩散加权成像（ DWI）对胃癌T分期的检测准确率。方法对59例经手术病理证实的胃癌患者进行常规MRI及DWI检查，以病理结果为金标准，比较常规MRI和DWI对胃癌T分期的检测准确率。结果胃癌病变T2 WI呈等或稍高信号，DWI呈明显高信号。常规MRI、DWI最终检测到所有病灶。 DWI总体分期准确率（83．0％）显著高于常规MRI（69．5％）（P＝0．003），DWI对T3期肿瘤诊断准确率（60．0％）亦显著高于常规MRI（88．0％）（P＝0．02）。结论 DWI联合常规MRI检查，不仅有利于胃癌的检测，而且能提高胃癌T分期准确率，具有一定的临床实用价值。
목적：탐토확산가권성상（ DWI）대위암T분기적검측준학솔。방법대59례경수술병리증실적위암환자진행상규MRI급DWI검사，이병리결과위금표준，비교상규MRI화DWI대위암T분기적검측준학솔。결과위암병변T2 WI정등혹초고신호，DWI정명현고신호。상규MRI、DWI최종검측도소유병조。 DWI총체분기준학솔（83．0％）현저고우상규MRI（69．5％）（P＝0．003），DWI대T3기종류진단준학솔（60．0％）역현저고우상규MRI（88．0％）（P＝0．02）。결론 DWI연합상규MRI검사，불부유리우위암적검측，이차능제고위암T분기준학솔，구유일정적림상실용개치。
Objective To study of detection accuracy of MRI and diffusion-weighted imaging ( DWI ) for T staging of gastric cancer.Methods 59 cases of patients with pathologically confirmed gastric cancer received conventional MRI and DWI examination,and the pathological findings as the gold standard ,detective accuracy of T staging of gastric cancer by conventional MRI and DWI were compared .Results Gastric lesions in the T 2 WI showed equal or slightly higher signal ,DWI showed a high signal.Conventional MRI and DWI eventually detected all lesions .DWI overall staging accuracy rate (83.0%) was significantly higher than that of conventional MRI (69.5%) (P=0.003),diagnostic accuracy of T3 tumors by DWI (60.0%) was signifi-cantly higher than that of conventional MRI (88.0%) (P=0.02).Conclusion DWI combined with conventional MRI detec-tion of gastric cancer is beneficial for gastric cancer detection ,and improves the accuracy of T staging ,it has certain clinical val-ue.
