目的：探讨血管内皮生长因子（ VEGF）在肾母细胞瘤血管生成中的作用。方法选取2006年1月至2012年1月河北医科大学第二医院小儿外科收治的30例肾母细胞瘤中保存完好的肿瘤组织（包括手术切除和活检）和血清，按美国肾母细胞瘤研究组（NWTSG）标准进行分期，其中Ⅰ期4例，Ⅱ期5例，Ⅲ期18例，Ⅳ期3例。分为Ⅰ＋Ⅱ期和Ⅲ＋Ⅳ期2组。应用免疫组织化学方法检测肿瘤组织中VEGF表达和MVD，采用ELISA法检测各组患儿血清血管内皮生长因子，选取20例对照组患儿，并应用统计学方法进行分析。结果肾母细胞瘤VEGF及MVD水平在患儿不同年龄、性别间差异无统计学意义（ P ＜0．05）；Ⅲ＋Ⅳ期瘤体中VEGF表达及MVD均高于Ⅰ＋Ⅱ期（ P ＜0．05）；瘤体中VEGF表达与MVD呈正相关（ r ＝0．672， P ＝0．00047）。结论瘤体中VEGF表达可反映肿瘤MVD；瘤体中VEGF表达和MVD以及血清中VEGF水平均可大致反映肿瘤分期；血清VEGF可能由瘤体分泌。
목적：탐토혈관내피생장인자（ VEGF）재신모세포류혈관생성중적작용。방법선취2006년1월지2012년1월하북의과대학제이의원소인외과수치적30례신모세포류중보존완호적종류조직（포괄수술절제화활검）화혈청，안미국신모세포류연구조（NWTSG）표준진행분기，기중Ⅰ기4례，Ⅱ기5례，Ⅲ기18례，Ⅳ기3례。분위Ⅰ＋Ⅱ기화Ⅲ＋Ⅳ기2조。응용면역조직화학방법검측종류조직중VEGF표체화MVD，채용ELISA법검측각조환인혈청혈관내피생장인자，선취20례대조조환인，병응용통계학방법진행분석。결과신모세포류VEGF급MVD수평재환인불동년령、성별간차이무통계학의의（ P ＜0．05）；Ⅲ＋Ⅳ기류체중VEGF표체급MVD균고우Ⅰ＋Ⅱ기（ P ＜0．05）；류체중VEGF표체여MVD정정상관（ r ＝0．672， P ＝0．00047）。결론류체중VEGF표체가반영종류MVD；류체중VEGF표체화MVD이급혈청중VEGF수평균가대치반영종류분기；혈청VEGF가능유류체분비。
Objective To investigate the significance of vascular endothelial growth factor ( VEGF) and microvessel density ( MVD) in pathogenesis of nephroblastoma .Methods Thirty specimens of nephroblastoma tissues including surgical resection,biopsy and serum of patients with nephroblastoma hospitalized from January 2006 to January 2013 were enrolled in the study,and these specimens were divided into I ~IV stage groups and the other two groups (Ⅰ+ⅡandⅢ+Ⅳ) according to NWTSG criteria (National Wilms’ Tumor Study Group),in which there were 4 cases at stage,5 cases atⅡstage,18 cases atⅢstage,3 cases at Ⅳstage.The expressions of VEGF and MVD in tumor tissue were detected by immunohistochemistry . The other 20 cases of nonnephroblastoma were selected as control group .Results There were no significant differences in the expression levels of VEGF and MVD in tumor tissue of children patients among different age groups and between males and females ( P >0.05).The expression levels of VEGF and MVD in tumor tissue at Ⅲ+Ⅳstage were significant higher than those at Ⅰ+Ⅱstage ( P <0.05).The expression of VEGF in tumor tissue was positively correlated with that of MVD ( r =0.672, P <0.01).Conclusion The expression of VEGF in tumor tissue can reflect the condition of MVD;the expression of VEGF,MVD in tumor tissue and serum can reflect the tumor stageing;sVEGF may be secreted from the tumors .
