目的：探讨自制弹性弧形骨钻在椎体后凸成形术（PKP）治疗骨质疏松性椎体压缩性骨折中的安全性、疗效及适应证的选择。方法：应用自制弹性弧形骨钻行PK P治疗骨质疏松性椎体压缩骨折28例（35个椎体）。比较手术前、后视觉模拟法（VAS）评分、伤椎前缘高度及伤椎Cobb角变化。结果：28例患者术后24h均可带腰围下地活动，VAS评分、伤椎前缘高度及Cobb角均较术前明显恢复，差异均有统计学意义（P＜0．05）。术中单侧椎弓根穿刺置入球囊位置良好，骨水泥分布均衡，无椎弓根穿破的患者，围手术期无肺栓塞、神经损伤等并发症发生。结论：应用弹性弧形骨钻在椎体后凸成形术治疗骨质疏松性椎体压缩性骨折是安全有效的，可以实现单侧椎弓根穿刺骨水泥双侧均衡分布的目的，简化穿刺过程，手术疗效肯定。
목적：탐토자제탄성호형골찬재추체후철성형술（PKP）치료골질소송성추체압축성골절중적안전성、료효급괄응증적선택。방법：응용자제탄성호형골찬행PK P치료골질소송성추체압축골절28례（35개추체）。비교수술전、후시각모의법（VAS）평분、상추전연고도급상추Cobb각변화。결과：28례환자술후24h균가대요위하지활동，VAS평분、상추전연고도급Cobb각균교술전명현회복，차이균유통계학의의（P＜0．05）。술중단측추궁근천자치입구낭위치량호，골수니분포균형，무추궁근천파적환자，위수술기무폐전새、신경손상등병발증발생。결론：응용탄성호형골찬재추체후철성형술치료골질소송성추체압축성골절시안전유효적，가이실현단측추궁근천자골수니쌍측균형분포적목적，간화천자과정，수술료효긍정。
Objective:To evaluate the effict that homemade elastic arc bone drill was used in percutaneous kyphoplasty (PKP) for osteoporotic vertebral compression fractures (OVCFs) in safety ,efficacy and indication . Methods :28 cases who sustained osteoporotic vertebral compression fracture were treated by PKP that used by Homemade elastic arc bone drill .The cases were including 35 vertebral body .The VAS scores improvement ,anteri-or vertebral height restoration and angle of Cobb were compared between between preoperative and postoperative . Results :The 28 cases were cloud waking by protection of waistband after 24 hours of operation ,the VAS score de-creased significantly after operation .28 cases had a significant recovery for average vertebral height and local Cobb ’s angle in postoperative .The PKP were operated via unilateral approach .The location of balloon were well .The distri-bution of bone cements were equilibrium .The vertebral pedicles were not broken in operation ,the complications in-cluding nerve damage and pulmonary embolism were disappearance .Conclusion:Homemade elastic arc bone drill in the application of PKP remain a reliable and effective treatment for OVCFs .The cement distribute evenly in PKP by used Homemade elastic arc bone drill .Simplify the procedure of puncture .
