目的：分析腹腔镜辅助下胃癌根治切除术治疗进展期胃癌的疗效。方法中国医学科学院肿瘤医院2009年1月至6月按纳入标准收治的进展期胃癌患者164例，根据手术方式分为实验组（腹腔镜组）和对照组（开腹组），其中实验组79例，对照组85例。实验组除使用腹腔镜辅助下胃癌根治切除术外，其他治疗同对照组。比较术中各项指标、术后病理学结果及恢复状况、并发症、5年生存率。结果实验组临床一般情况恢复明显好于对照组，差异有统计学意义（P＜0．05）。两组严重并发症发生率差异有统计学意义（P＜0．05）。两组5年生存率差异无统计学意义（P＞0．05）。结论腹腔镜下辅助胃癌根治切除术治疗进展期胃癌是安全可行的，能获得与开腹手术相当的治疗效果，且术后恢复有明显优势。
목적：분석복강경보조하위암근치절제술치료진전기위암적료효。방법중국의학과학원종류의원2009년1월지6월안납입표준수치적진전기위암환자164례，근거수술방식분위실험조（복강경조）화대조조（개복조），기중실험조79례，대조조85례。실험조제사용복강경보조하위암근치절제술외，기타치료동대조조。비교술중각항지표、술후병이학결과급회복상황、병발증、5년생존솔。결과실험조림상일반정황회복명현호우대조조，차이유통계학의의（P＜0．05）。량조엄중병발증발생솔차이유통계학의의（P＜0．05）。량조5년생존솔차이무통계학의의（P＞0．05）。결론복강경하보조위암근치절제술치료진전기위암시안전가행적，능획득여개복수술상당적치료효과，차술후회복유명현우세。
Objective To study the effect of laparoscopicassisted radical gastrectomy in advanced gastric cancer.Methods According to the inclusion criteria,164 patients with advanced gastriccancer who treated in the Cancer Institute and Hospital,Chinese Academy of Medical Sciences betweenJanuary and June of 2009 were enrolled into the study.Based on the operation methods,all cases were divided into the experimental group(laparoscopy group,n =79)and the control group(laparotomy group,n =85).Except for the laparoscopicassisted radical gastrectomy,patients received the same treatment in thethe experimental group and control group.Intraoperative indexes,postoperative pathological results and recovery,complications and 5year survival rate were observed and compared between groups.Results Therecovery time of patient's general condition in the experimental group was shorter than that of the controlgroup(P <0.05).The difference of serious postoperative complications rate between the two groups wassignificant(P <0.05).The difference of 5year survival rate between the two groups was not significant(P >0.05).Conclusion Laparoscopicassisted radical gastrectomy is safe and feasible in the treatmentof advanced gastric cancer.This technique could get equal treatment effect as laparotomy and it has obviousadvantage in postoperative recovery.
