目的研究血浆脑钠肽(BNP)与急性肺栓塞(APE)患者右心室功能不全(RVD)的关系及其预后评估价值。方法采用酶联免疫法测定44例APE患者血浆BNP水平,同时根据超声心动图检查分为RVD组和无RVD组。观察两组间血浆BNP水平差异及血浆BNP水平与不良事件发生率的关系。结果 APE患者RVD组BNP水平[(2352.5±1987.8)pg/ml]高于无RVD组[(349.9±299.4)pg/ml](P<0.01);低BNP者不良事件发生率(10.0%)低于高BNP者(69.2%),差异有统计学意义(P<0.05);低BNP者1年生存率高于高BNP者(χ2=6.554,P<0.05)。结论 BNP可作为反应APE患者RVD的有效手段,对预测患者不良事件及预后有重要作用。

목적연구혈장뇌납태(BNP)여급성폐전새(APE)환자우심실공능불전(RVD)적관계급기예후평고개치。방법채용매련면역법측정44례APE환자혈장BNP수평,동시근거초성심동도검사분위RVD조화무RVD조。관찰량조간혈장BNP수평차이급혈장BNP수평여불량사건발생솔적관계。결과 APE환자RVD조BNP수평[(2352.5±1987.8)pg/ml]고우무RVD조[(349.9±299.4)pg/ml](P<0.01);저BNP자불량사건발생솔(10.0%)저우고BNP자(69.2%),차이유통계학의의(P<0.05);저BNP자1년생존솔고우고BNP자(χ2=6.554,P<0.05)。결론 BNP가작위반응APE환자RVD적유효수단,대예측환자불량사건급예후유중요작용。
Objective To investigate the correlation of plasma B type natriuretic peptide (BNP) level with right ventricular function in patients with acute pulmonary embolism. Methods Plasma BNP level was measured by ELISA in 44 patients with a-cute pulmonary embolism. According to the results of echocardiography,the patients were classified as normal right ventricular function group and right ventricular dysfunction group.The correlation of plasma BNP level with ventricular function and the inci-dence of adverse events was analyzed. Results BNP level in right ventricular dysfunction group were significantly higher than that in normal right ventricular function group (2 352.5±1 987.8 vs 349.9±299.4 pg/ml, P<0.01).The incidence of adverse out-come events in the BNP- positive group was higher than that in BNP- negative group (69.2%vs 10.0%, P<0.05).The survival rate in the BNP- negative group was higher than that in BNP- positive group (χ2=6.554, P<0.05). Conclusion Plasma BNP lev-els are correlated with right ventricular function in patients with acute pulmonary embolism, which may be of prognostic value for acute pulmonary embolism.
