目的观察原发性高血压患者运动时血压水平与高敏C反应蛋白(hs- CRP)和踝臂脉搏波传导速度(BaPWV)的相关性。方法选择高血压病1、2级患者共80例,均行血脂、血糖、hs- CRP和BaPWV检测,然后所有受试者均接受运动平板试验,分别测量运动前、运动亚极量、运动后的血压情况,按照运动高血压的标准,将受试者分为运动高血压组(H组)和运动血压正常组(N组),比较两组间收缩压(SBP)、舒张压(DBP)、脉压(PP)、hs- CRP和BaPWV等指标,以及运动血压水平与hs- CRP、BaPWV的相关性。结果 H组hs- CRP浓度和BaPWV值[(8.42±3.82)mg/L、(19.84±8.13)m/s]均高于N组[(6.64±4.12)mg/L、(15.71±9.24) m/s](均P<0.05),并与运动亚极量时SBP、PP水平具有相关性(hs- CRP与SBP、PP,r=0.26、0.25,均P<0.05;BaPWV与SBP、PP,r=0.29、0.36,P<0.05或0.01)。结论运功高血压患者较运动血压正常者炎症反应和动脉硬化程度更明显,对运动高血压的早期发现和积极干预,可能在一定程度上能改善高血压患者的预后。

목적관찰원발성고혈압환자운동시혈압수평여고민C반응단백(hs- CRP)화과비맥박파전도속도(BaPWV)적상관성。방법선택고혈압병1、2급환자공80례,균행혈지、혈당、hs- CRP화BaPWV검측,연후소유수시자균접수운동평판시험,분별측량운동전、운동아겁량、운동후적혈압정황,안조운동고혈압적표준,장수시자분위운동고혈압조(H조)화운동혈압정상조(N조),비교량조간수축압(SBP)、서장압(DBP)、맥압(PP)、hs- CRP화BaPWV등지표,이급운동혈압수평여hs- CRP、BaPWV적상관성。결과 H조hs- CRP농도화BaPWV치[(8.42±3.82)mg/L、(19.84±8.13)m/s]균고우N조[(6.64±4.12)mg/L、(15.71±9.24) m/s](균P<0.05),병여운동아겁량시SBP、PP수평구유상관성(hs- CRP여SBP、PP,r=0.26、0.25,균P<0.05;BaPWV여SBP、PP,r=0.29、0.36,P<0.05혹0.01)。결론운공고혈압환자교운동혈압정상자염증반응화동맥경화정도경명현,대운동고혈압적조기발현화적겁간예,가능재일정정도상능개선고혈압환자적예후。
Objective To investigate of the exercise blood pressure with serum high- sensitivity C- reactive protein ( hs- CRP ) levels and brachial- ankle pulse wave conduction velocity ( BaPWV ) in essential hypertension patients . Meth-ods Eighty patients with essential hypertension (level 1 and 2) were enrol ed in the study. Blood lipids, blood glucose(GLU), hs- CRP were measured and BaPWV was detected. Treadmil exercise testing was performed and the blood pressure was measured before, during and after exercise. The patients were identified as exercise hypertension group (group H) and normal blood pressure group(group N) according to exercise blood pressure. The systolic blood pressure(SBP), diastolic blood pres-sure (DBP), pulse pressure (PP), hs- CRP and BaPWV were compared between two groups; the correlation of exercise blood pressure level with hs- CRP, BaPWV was analyzed. Results The hs- CRP level and BaPWV value of group H were significantly higher than those of group N (hs- CRP 8.42±3.82 vs 6.64±4.12mg/L, P<0.05;BaPWV 19.84±8.13 vs 15.71±9.24m/s, P<0.05). hs- CRP level was correlated with SBP and PP values (r=0.25 and 0.26, P<0.05), BaPWV was correlated with SBP and PP values (r=0.29 and 0.36, P<0.05 or 0.01). Conclusion Patients with exercise hypertension have more severe inflammatory response and arteriosclerosis than patients with normal exercise blood pressure.
