目的:探讨和评估阻挡螺钉技术在带锁髓内钉内固定治疗下肢长骨干骺端骨折的临床疗效和注意事项。方法:2008年1月-2009年12月应用带锁髓内钉内固定结合阻挡螺钉技术治疗下肢长骨干骺端26例,其中胫骨远端骨折15例,股骨远端11例;闭合性骨折23例,开放性骨折3例。术后通过随访评估骨折复位质量以及愈合情况。结果:本组26例共应用阻挡螺钉38枚,26例均获得随访,随访时间12~18个月,平均15.2个月。所有病例骨折均为骨性愈合,平均愈合时间为5.1个月。所有骨折愈合成角小于5°,未见骨折移位和断端不稳定现象,未发生常见的软组织并发症如严重肿胀、感染等现象。阻挡螺钉及整个内固定系统位置稳定,未出现内固定物松动、变形、移位、断裂等并发症。结论:阻挡螺钉技术在带锁髓内钉治疗下肢长骨干骺端骨折中能够有效提升骨折复位的质量、增加骨折固定的稳定性,从而扩大髓内钉的应用范围。
目的:探討和評估阻擋螺釘技術在帶鎖髓內釘內固定治療下肢長骨榦骺耑骨摺的臨床療效和註意事項。方法:2008年1月-2009年12月應用帶鎖髓內釘內固定結閤阻擋螺釘技術治療下肢長骨榦骺耑26例,其中脛骨遠耑骨摺15例,股骨遠耑11例;閉閤性骨摺23例,開放性骨摺3例。術後通過隨訪評估骨摺複位質量以及愈閤情況。結果:本組26例共應用阻擋螺釘38枚,26例均穫得隨訪,隨訪時間12~18箇月,平均15.2箇月。所有病例骨摺均為骨性愈閤,平均愈閤時間為5.1箇月。所有骨摺愈閤成角小于5°,未見骨摺移位和斷耑不穩定現象,未髮生常見的軟組織併髮癥如嚴重腫脹、感染等現象。阻擋螺釘及整箇內固定繫統位置穩定,未齣現內固定物鬆動、變形、移位、斷裂等併髮癥。結論:阻擋螺釘技術在帶鎖髓內釘治療下肢長骨榦骺耑骨摺中能夠有效提升骨摺複位的質量、增加骨摺固定的穩定性,從而擴大髓內釘的應用範圍。
목적:탐토화평고조당라정기술재대쇄수내정내고정치료하지장골간후단골절적림상료효화주의사항。방법:2008년1월-2009년12월응용대쇄수내정내고정결합조당라정기술치료하지장골간후단26례,기중경골원단골절15례,고골원단11례;폐합성골절23례,개방성골절3례。술후통과수방평고골절복위질량이급유합정황。결과:본조26례공응용조당라정38매,26례균획득수방,수방시간12~18개월,평균15.2개월。소유병례골절균위골성유합,평균유합시간위5.1개월。소유골절유합성각소우5°,미견골절이위화단단불은정현상,미발생상견적연조직병발증여엄중종창、감염등현상。조당라정급정개내고정계통위치은정,미출현내고정물송동、변형、이위、단렬등병발증。결론:조당라정기술재대쇄수내정치료하지장골간후단골절중능구유효제승골절복위적질량、증가골절고정적은정성,종이확대수내정적응용범위。
Objective:To investigate and assessment the clinical efficacy and notes in the treatment of interlocking intramedullary nail with the blocking screws fixation of lower extremity long bone metaphyseal fractures.Methods:From January 2008 to December 2009,26 cases of lower extremity long bone metaphyseal fractures were surgical treatment and used the interlocking intramedullary nail fixation combined with blocking screws,in them including 15 cases of distal tibial fractures,11 cases of distal femur;Closed fractures were 23 cases and open fractures were 3 cases.Postoperative follow-up assessment of quality of fracture reduction and healing.Results:38 blocking screws were used in 26 cases,26 cases were all followed up for 12-18 months,with an average of 15.2 months.All the fractures were bone union,the average healing time was 5.1 months.All fracture healing angle less than 5°,no fracture shift and ends instability phenomenon,not fond common soft tissue complications such as severe swelling,infection and other phenomena.The blocking screw and interlocking intramedullary nail fixation system were stable position,did not appear the complications such as internal fixation loosening,deformation,displacement and fracture.Conclusion:Blocking screw technology can effectively enhance the quality of fracture reduction in lower extremity long bone metaphyseal fractures treated with interlocking nails,and increase the stability of fracture fixation,thereby expanding the range applications of the intramedullary nail.
