【目的】探讨噻托溴铵联合舒利迭治疗老年中重度慢性阻塞性肺疾病（COPD ）的临床疗效。【方法】本院住院的46例老年中重度COPD患者随机分为试验组和对照组，每组23例，患者在COPD常规治疗的基础上分别接受噻托溴铵联合舒利迭治疗（试验组）和单用舒利迭治疗（对照组），观察并比较两组治疗6个月后的临床症状积分、肺功能和动脉血气分析变化。【结果】两组治疗后临床症状评分均较治疗前显著降低，肺功能、动脉血气分析则明显改善，治疗前后比较差异均有显著性（ P ＜0．05）；治疗后试验组临床症状评分、肺功能和动脉血气分析较对照组改善更为明显，且组间比较差异均有显著性（ P ＜0．05）。【结论】噻托溴铵联合舒利迭治疗老年中重度COPD疗效满意，安全性高，值得临床推广应用。
【목적】탐토새탁추안연합서리질치료노년중중도만성조새성폐질병（COPD ）적림상료효。【방법】본원주원적46례노년중중도COPD환자수궤분위시험조화대조조，매조23례，환자재COPD상규치료적기출상분별접수새탁추안연합서리질치료（시험조）화단용서리질치료（대조조），관찰병비교량조치료6개월후적림상증상적분、폐공능화동맥혈기분석변화。【결과】량조치료후림상증상평분균교치료전현저강저，폐공능、동맥혈기분석칙명현개선，치료전후비교차이균유현저성（ P ＜0．05）；치료후시험조림상증상평분、폐공능화동맥혈기분석교대조조개선경위명현，차조간비교차이균유현저성（ P ＜0．05）。【결론】새탁추안연합서리질치료노년중중도COPD료효만의，안전성고，치득림상추엄응용。
[Objective] To explore clinical efficacy of tiotropium combined with seretide for the treatment of elderly patients with severe chronic obstructive pulmonary disease (COPD) .[Methods] Totally 46 elderly patients with moderate to severe COPD were randomly divided into experimental group and control group with 23 patients in each group .Patients received tiotropium combined with seretide (experimental group) and simple seretide respectively on the basis of conventional therapy .Clinical symptom score ,lung function and arterial blood gas analysis changes in two groups 6 month after treatment were observed and compared .[Results]Compared with before treatment ,clinical symptom score of both groups after treatment decreased significant-ly ,while lung function and arterial blood gas analysis improved significantly ,and there were significant differ-ences between before and after treatment ( P <0 .05) .After treatment ,clinical symptom score ,lung function and arterial blood gas analysis of experimental group improved more obvious than the control group ,and there was significant difference between two groups ( P <0 .05) .[Conclusion] Tiotropium combined with seretide for the treatment of elderly patients with moderate to severe COPD has satisfactory efficacy and high safety ,so it is worthy of clinical application .
