目的:探讨异位妊娠发生破裂的相关危险因素,为异位妊娠的治疗提供参考依据。方法采用回顾性病例对照研究对65例异位妊娠发生破裂患者和118例异位妊娠未发生破裂患者的临床资料进行分析,单因素分析采用卡方检验,多因素分析采用非条件性Logistic回归法。结果产次(OR=5.362,95%CI:0.843~9.785)、妊娠部位(OR=3.757,95%CI:0.458~6.857)、异位妊娠史(OR=3.785,95%CI:0.753~5.978)、异位妊娠史包块直径(OR=1.478,95%CI:0.742~2.424)是异位妊娠破裂的相关危险因素。结论妊娠部位、异位妊娠史、包块直径是异位妊娠破裂的相关危险因素,临床工作中应对具有上述危险因素的异位妊娠患者早期提高警惕性,避免发生破裂大出血导致严重的不良预后。

Objective To investigate the risk factors of ruptured ectopic pregnancy ( EP) and to provide reference for the treatment of EP . Methods Retrospective case control study was used to analyze the clinical data of 118 cases of unruptured EP and 65 cases of ruptured EP . The statistical relationship was analyzed by using chi-square test and multiple unconditional Logistic regression method .Results The parity (OR=5.362, 95%CI:0.843-9.785), site of pregnancy(OR=3.757, 95%CI:0.458-6.857), history of EP (OR=3.785, 95%CI:0.753-5.978), and mass diameter of EP (OR=1.478,95%CI:0.742-2.424) were related risk factors of EP rupture .Conclusion Site of pregnancy , history of EP and mass diameter are the related risk factors of EP rupture .In clinical work close attention should be paid to EP patients with the above risk factors so as to avoid rupture bleeding and serious adverse outcomes .
