目的讨论白内障手术中对伴有无张力虹膜白内障小切口摘出手术方法.方法应用小切口白内障摘出手术中发现无张力虹膜19例(19眼).表现为虹膜软如丝绸,手术中虹膜能随注吸液从隧道主切口、辅助切口溢出,亦可从晶状体核排出时同时脱出于切口外.用黏弹剂推(把脱出的虹膜推入前房)、堵(用黏弹剂堵塞隧道切口和辅助切口)、压(用黏弹剂下压虹膜和后囊),封(手术完成后用黏弹剂注射封于辅助切口和主切口区,防止术后虹膜脱出)等方法,阻止虹膜脱出和还纳虹膜复位.结果经以上方法处理,手术后瞳孔轻度上移2例(2眼),为手术中反复复位虹膜致色素脱失所致；余病例效果良好.手术后视力0.3 ～1.0.结论小切口白内障手术中遇到无张力虹膜时,应用黏弹剂推压封堵等方法,可顺利完成手术.

Objective To discuss the method of manual small incision cataract surgery (MSICS) in cataract with inelastic iris.Methods 19 cataract patients with inelastic iris were found in operation with MSICS.The iris was soft like silk,which can overflow with the perfusate from the main incision and assistant incision,or emersion from the incision with the lens extraction.The management including using the sodium hyaluronate for pushing the iris into the anterior chamber,occluding incision,depressing the iris and postcapsule for blocking out the iris emersion from incision after operation.Results 2 cases had mild corectopia after operation.The remaining cases were good in iris shape and position.The postoperative visual acuity was 0.3 ～ 1.0.Conclusion The cataract operation with inelastic iris can be accomplished well-off according to the above-mentioned methods of using the sodium hyaluronate.
