目的:了解张家港市6月龄以内婴儿髋关节发育状况及超声筛查发育性髋关节发育不良( DDH)的检出率,探讨筛查模式的可行性,以便早期诊断、干预,降低整体人群发育性髋关节发育不良发病率与致残率。方法利用妇幼保健三级网络,对张家港市7830名6月龄以内的婴儿进行髋关节超声检查与流行病学调查。结果超声筛查检出率为4.15%(325/7830),其中女婴发育性髋关节发育不良检出率明显高于男婴(χ2=67.64,P<0.05);左侧发生率高于右侧(χ2=5.15,P<0.05);秋季出生婴儿髋关节超声筛查结果异常率高于夏、春、冬季(χ2=68.46,P<0.05)。经随访有10名婴儿临床诊断为先天性髋关节脱位。结论以政府为主导,利用妇幼三级网络早期筛查婴儿发育性髋关节异常值得推广。
目的:瞭解張傢港市6月齡以內嬰兒髖關節髮育狀況及超聲篩查髮育性髖關節髮育不良( DDH)的檢齣率,探討篩查模式的可行性,以便早期診斷、榦預,降低整體人群髮育性髖關節髮育不良髮病率與緻殘率。方法利用婦幼保健三級網絡,對張傢港市7830名6月齡以內的嬰兒進行髖關節超聲檢查與流行病學調查。結果超聲篩查檢齣率為4.15%(325/7830),其中女嬰髮育性髖關節髮育不良檢齣率明顯高于男嬰(χ2=67.64,P<0.05);左側髮生率高于右側(χ2=5.15,P<0.05);鞦季齣生嬰兒髖關節超聲篩查結果異常率高于夏、春、鼕季(χ2=68.46,P<0.05)。經隨訪有10名嬰兒臨床診斷為先天性髖關節脫位。結論以政府為主導,利用婦幼三級網絡早期篩查嬰兒髮育性髖關節異常值得推廣。
목적:료해장가항시6월령이내영인관관절발육상황급초성사사발육성관관절발육불량( DDH)적검출솔,탐토사사모식적가행성,이편조기진단、간예,강저정체인군발육성관관절발육불량발병솔여치잔솔。방법이용부유보건삼급망락,대장가항시7830명6월령이내적영인진행관관절초성검사여류행병학조사。결과초성사사검출솔위4.15%(325/7830),기중녀영발육성관관절발육불량검출솔명현고우남영(χ2=67.64,P<0.05);좌측발생솔고우우측(χ2=5.15,P<0.05);추계출생영인관관절초성사사결과이상솔고우하、춘、동계(χ2=68.46,P<0.05)。경수방유10명영인림상진단위선천성관관절탈위。결론이정부위주도,이용부유삼급망락조기사사영인발육성관관절이상치득추엄。
Objective To understand the hip development condition and the prevalence of developmental dysplasia of the hip (DDH) measured by ultrasound among infants within 6 months in Zhangjiagang and to explore the feasibility of screening model , so as to provide evidence for early diagnosis and intervention and to reduce the incidence and mutilation rate of DDH in overall population .Methods Ultrasound examination and epidemiological investigation were taken among 7 380 infants in Zhangjiagang by three-level maternal and child health network.Results The detection rate of DDH was 4.15%(325/7 830).The detection rate of DDH in female infants was significantly higher than that in male infants (χ2 =67.64,P<0.05), and the detection rate of developmental dysplasia of left hip was significantly higher than that of right one(χ2 =5.15,P<0.05).Compared with summer, spring and winter, the detection rate of DDH in infants born in autumn was higher (χ2 =68.46,P<0.05).Ten infants were diagnosed as congenital dislocation of hip after following up .Conclusion Under leading of government , screening DDH in infants with three-level maternal and child network is worthy of promotion .
