目的:讨论无创双水平正压通气在重度支气管哮喘发作治疗时对pH值、PaO2、PaCO2、临床症状及住院时间的影响,进而判断无创通气对重症支气管哮喘的治疗效果。方法选择41例明确诊断为重度支气管哮喘急性发作的患者,将41例患者随机分为试验组(21例)和对照组(20例),试验组和对照组均常规给予氧疗、支气管扩张剂、糖皮质激素、抗感染等治疗,试验组同时加用面罩无创通气(BiPAP)治疗。观察两组治疗前后患者动脉血气分析、症状缓解程度、住院时间等指标的变化。结果实验组治疗2h及72 h后pH值、PaO2值上升和PaCO2值降低,平均住院时间缩短,较对照组差异有统计学意义(P<0.05)。结论:双水平气道正压通气是治疗支气管哮喘的有效方法,在重度支气管哮喘急性发作治疗中可改善呼吸困难及氧合情况,缩短患者住院时间,疗效显著。

Objectiveve To investigate the value of application of of noninvasive BiPAP ventilation in patients with severe asthma.Methods Selected 41 patients diagnosed with severe asthma were randomly divided into two groups,trial group(21 patients) and control group(20 patients).Both the two groups had been given routine treatment for asthma(such as oxygen therapy,bronchial dilators,steroids and antibiotics).In addition to routine treatment,the patients in trial group had also been given noninvasive BiPAP ventilation since the diagnosis was established.The changes of blood gas analysis (PH,PaO2,PCO2),symptoms and durations of hospitaliazation after treatment in two groups were observed and analyzed.Results The values of PH and PO2 increased,the values of PCO2 decreased at 2 hours and 72 hours after initiation of ventilation.The tiral group showed significant differences as compared with control group(al P<O.05).The durations of hospitalization in the trial group were obviously shortened compared to the control group.Conclusion The noninvasive BiPAP ventilation is a effective way to treat severe asthma.It can improve arterial blood gas analysis,relieve symptoms and shorten durations of hospitalization.
