目的：探讨基础胰岛素与预混胰岛素控制围手术期血糖的疗效与安全性临床比较情况。方法分析我院80例择期中、大型手术2型糖尿病患者临床资料，依据围手术期降糖方案不同进行临床分组，对照组（预混胰岛素组）40例和观察组（基础胰岛素组）40例。结果观察组糖尿病患者三餐餐前、餐后2 h、睡前22时及凌晨3时血糖、血糖达标时间、每天胰岛素用量、低血糖发生率、切口感染或延迟愈合率均明显低于对照组，P ＜0.05，差异均有统计学意义。结论基础胰岛素控制围手术期血糖是一种安全、有效、快速、平稳的胰岛素降糖方案，可作为糖尿病患者围手术期胰岛素治疗方案进行推广。
목적：탐토기출이도소여예혼이도소공제위수술기혈당적료효여안전성림상비교정황。방법분석아원80례택기중、대형수술2형당뇨병환자림상자료，의거위수술기강당방안불동진행림상분조，대조조（예혼이도소조）40례화관찰조（기출이도소조）40례。결과관찰조당뇨병환자삼찬찬전、찬후2 h、수전22시급릉신3시혈당、혈당체표시간、매천이도소용량、저혈당발생솔、절구감염혹연지유합솔균명현저우대조조，P ＜0.05，차이균유통계학의의。결론기출이도소공제위수술기혈당시일충안전、유효、쾌속、평은적이도소강당방안，가작위당뇨병환자위수술기이도소치료방안진행추엄。
Objective To approach the clinical comparison of control efficacy and safety of perioperative period of blood glucose by basal insulin and premixed insulin. Method To analyze 80 cases clinical data of 2 DM patients undergoing elective operation in our hospital, which was to be divided into control group (premixed insulin group) 40 cases and observation group (basal insulin group) 40 cases. Result The Before the meal, 2 hours after meals of Three meals, before going to bed at 22 and at 3 in the morning the blood sugar、recovery time, daily insulin dosage, incidence of hypoglycemia、Incision infection or delayed healing rate of DM patients of observation group were lower than control group, P<0.05,the difference were statistical significance. Conclusion The basal insulin to control blood glucose during peri operation was Insulin therapy is safe and effective, fast, stable, which was was to be used for Diabetic patients with peri operation period of insulin therapy.
