目的：探讨血清同型半胱氨酸检测在T2DM早期肾损伤中临床应用价值。方法选取2009年10月～2013年6月本院进行治疗的115例T2DM患者病程、血清同型半胱氨酸、糖化血红蛋白、尿微量白蛋白、血肌酐含量进行检测，根据mALB量将DM患者分为三组，以同期在本院进行体检的40例健康人群作为正常对照组，并对四组检测结果进行比较。结果 T2DM患者血清Hcy含量高于健康对照组，各组间差异具有统计学意义（P＜0.05或P＜0.01）；血清Hcy含量与病程、mALB、CREA水平呈正相关（r分别为0.66、0.59、0.46），P＜0.05。结论随着T2DM患者血清Hcy水平增加，尿mALB水平逐渐增加，提示血清Hcy和尿mALB联合检测可以动态观T2DM患者肾脏微血管病变程度，提高DM及EDN诊断效果，具有很好的临床应用价值。
목적：탐토혈청동형반광안산검측재T2DM조기신손상중림상응용개치。방법선취2009년10월～2013년6월본원진행치료적115례T2DM환자병정、혈청동형반광안산、당화혈홍단백、뇨미량백단백、혈기항함량진행검측，근거mALB량장DM환자분위삼조，이동기재본원진행체검적40례건강인군작위정상대조조，병대사조검측결과진행비교。결과 T2DM환자혈청Hcy함량고우건강대조조，각조간차이구유통계학의의（P＜0.05혹P＜0.01）；혈청Hcy함량여병정、mALB、CREA수평정정상관（r분별위0.66、0.59、0.46），P＜0.05。결론수착T2DM환자혈청Hcy수평증가，뇨mALB수평축점증가，제시혈청Hcy화뇨mALB연합검측가이동태관T2DM환자신장미혈관병변정도，제고DM급EDN진단효과，구유흔호적림상응용개치。
Objective Study of serum homocysteine detection in the clinical application value of early renal injury in T2DM.Methods 115 cases of T2DM patients, serum homocysteine treated in this hospital in 2009 October~2013 years 6 months, glycosylated hemoglobin, urinary albumin, serum creatinine were detected. According to the mALB DM patients were divided into three groups, 40 healthy people for medical examination in our hospital at the same period as the normal control group, and to compare the results of four groups of detection.Results The content of serum Hcy in patients with T2DM than in healthy controls, with a statistical significant difference between the groups (P<0.05 orP<0.01);serum Hcy content and course of disease, mALB, CREA levels were positively (r were 0.66, 0.59, 0.46),P<0.05.Conclusion With the increase of serum Hcy level in patients with T2DM, Urinary mALB levels gradualy increased. The results suggest that the serum levels of Hcy and urine mALB can dynamicaly view kidney patients T2DM micro vascular lesions, To improve DM and EDN diagnosis, Has good clinical application value.
