目的：观察丹参川芎嗪注射液联合胰激肽原酶治疗早期糖尿病肾病（ DN）的临床疗效。方法将76例早期DN患者随机分为2组，对照组37例予胰激肽原酶肌肉注射治疗，治疗组39例在对照组基础上加用丹参川芎嗪注射液静脉滴注治疗。2组均治疗2周。测定2组治疗前后血肌酐（ Cr ）、尿素氮（ BUN）、肾小球滤过率（ GFR）、胆固醇（ TC）、甘油三酯（ TG）及24 h尿蛋白定量的变化。结果对照组 Cr、TC、TG及24 h尿蛋白定量均较治疗前明显下降（P＜0．05），BUN及GFR变化比较差异均无统计学意义（P＞0．05）。治疗组治疗后Cr、BUN、GFR、TC、TG及24 h尿蛋白定量改变与本组治疗前比较差异均有统计学意义（P＜0．01），且与对照组治疗后比较差异有统计学意义（P＜0．05）。结论丹参川芎嗪注射液联合胰激肽原酶治疗早期DN，可降低患者Cr、BUN、24 h尿蛋白定量及血脂水平，升高GFR，改善肾功能。
목적：관찰단삼천궁진주사액연합이격태원매치료조기당뇨병신병（ DN）적림상료효。방법장76례조기DN환자수궤분위2조，대조조37례여이격태원매기육주사치료，치료조39례재대조조기출상가용단삼천궁진주사액정맥적주치료。2조균치료2주。측정2조치료전후혈기항（ Cr ）、뇨소담（ BUN）、신소구려과솔（ GFR）、담고순（ TC）、감유삼지（ TG）급24 h뇨단백정량적변화。결과대조조 Cr、TC、TG급24 h뇨단백정량균교치료전명현하강（P＜0．05），BUN급GFR변화비교차이균무통계학의의（P＞0．05）。치료조치료후Cr、BUN、GFR、TC、TG급24 h뇨단백정량개변여본조치료전비교차이균유통계학의의（P＜0．01），차여대조조치료후비교차이유통계학의의（P＜0．05）。결론단삼천궁진주사액연합이격태원매치료조기DN，가강저환자Cr、BUN、24 h뇨단백정량급혈지수평，승고GFR，개선신공능。
Objective To observe the effect of Danshen Chuanxiongqin injection combined with pancreatic kininogenase on the treatment of early diabetic nephropathy ( DN) patients .Methods 76 cases of early diabetic ne-phropathy patients were randomly divided into treatment group and control group .Patients in two groups were treated with pancreatic kallikrein 40 U,intramuscular injection,once a day.Patients in the treatment group were added with Danshen Chuanxiongqin injection 10 mL plus normal saline 250 mL,1 times/day,intravenous infusion ,2 weeks.The changes of serum creatinine (Cr),blood urea nitrogen (BUN),glomerular filtration rate (GFR),blood lipid (TC, TG) and 24 h urinary protein before and after treatment were measured .Results In the control group ,Cr,TC,TG and 24 h urine total protein after treatment were decreased significantly as compared with those before treatment ( P<0.05).There was no significant changes between before and after treatment on BUN and GFR in control group (P>0.05).The treatment group Cr,TC,TG,24 h urine total protein,BUN and GFR after treatment were decreased signifi-cantly as compared with those before treatment in treatment group (P<0.01).Compared with the control group ,the difference was statistically significant (P<0.05).Conclusion Danshen Chuanxiongqin injection combined with pancreatic kininogenase for treatment of early diabetic nephropathy can significantly decrease the Cr ,BUN,24 h urine protein content and lipid levels ,increase GFR and improve renal function .
